About this role

Job summary

As a Senior Case Worker, you will help adults with care and support needs make positive, lasting changes to their health and wellbeing through smoking cessation. Working with people who may face barriers to accessing mainstream services, including those with disabilities, long-term conditions, mental health needs and complex social circumstances, you'll provide personalised support that empowers individuals to achieve their smokefree goals and improve their quality of life.

Working within the Younger Adults Team, you'll collaborate with Social Workers, Public Health colleagues and partner organisations to embed smoking cessation into social care practice. Through compassionate, non-judgemental conversations, effective partnership working and tailored interventions, you'll help tackle health inequalities, reduce smoking-related harm and contribute to the ambitions of Smokefree Hampshire 2030.

Main duties of the job

Complete assessments, reviews and community-based visits to identify people who would benefit from smoking cessation support.Use strengths-based and motivational approaches to help people overcome barriers to quitting smoking.Make referrals to local stop smoking services, provide practical advice and signpost individuals to relevant support and community resources.Use person-centred approaches to raise awareness of the benefits of stopping smoking and encourage engagement with evidence-based support.Promote awareness of smoking cessation support across the service, sharing learning with colleagues and contributing to preventative practice and continuous improvement.
