This is a varied and active position where no two days are the same. You will play a vital role in helping patients manage their health, recover from illness and maintain independence. You will work closely with registered nurses and the wider multidisciplinary team, carrying out delegated care and developing your clinical skills. You will also have the opportunity to work independently, planning your visits and managing your time effectively across the local area. Key responsibilities include:
Supporting patients with delegated nursing care in their own homes Carrying out clinical tasks such as dressings and glucose monitoring following training and competency sign off Supporting holistic, evidence based care that meets each patients individual needs Maintaining accurate patient records using electronic systems Assisting with administrative tasks to support the team Planning and organizing your own daily visits across the communityThis role requires travel across the local area, so a UK driving license and access to a car is essential.
