Job summary
This is a part time post of 30 hours per week.
We are looking to recruit a highly motivated, enthusiastic, caring and compassionate nurse to a band 5 post within the Hospice Daye Services, at St Benedicts Hospice. The unit is part of an integrated Specialist Palliative Care Team that provide high quality palliative support to patients who have been diagnosed with a life limiting disease. The unit offers both physical and psychological support to patients and their families through MDT working.
The post holder will work as a part of a multi-professional team, providing skilled palliative care to a group of patients that visit the unit on a daily basis, providing comprehensive holistic assessments, emotional and physical support plans. Also providing patients with the opportunity to achieve their goals, whilst identifying and supporting family and carers.

Main duties of the job
The post will be a five day working week Monday to Friday, The working day is 09.00-17.00.
Aims of the service:
To provide symptom, psychological and social/carer support on a weekly basis over a set period of time within a secure welcoming environment. The patients receive a Nursing assessment on attendance, this may also require further referral or investigation into current health care plans.
The unit is multi-professional and is patient focused upon their current needs, each day the patients that are attending are discussed and implementation of intervention that may be required.
Roles and responsibilities:
Holistic care needs assessment, including symptom and psychological needs.
Engaging with patients on a 1:1 basis
Facilitate group work/sessions to empower, engage and educate patients to improve wellbeing.
Support Patients and families with future care planning
Develop, implement and evaluate programmes of care.
Active participation in MDT meetings
Supporting and developing junior members of the team.
You must have outstanding communication skills, to work alongside complex situations and to be able to engage with patients, families, carers and other health care professionals.

Person Specification
Physical Skills Essential
Ability to assist patients to move as per Trust Moving and Handling Policy.
Desirable
Trained in moving and handling procedures.
Qualifications Essential
Current NMC Registration (Level 1 adult nurse) or evidence of qualification if newly qualified. Degree in Nursing or evidence of working towards. Mentorship preparation.
Desirable
Palliative / Cancer care qualification. Communication skills training. Counseling qualification. Clinical Teaching experience. Teaching qualification.
Experience Essential
Recent experience of working as a registered nurse in an acute hospital or community healthcare setting.
Desirable
Hospice experience. Clinical decision making ability. 2 years post registration. Oncology experience. Experience with other life limiting illnesses. Managing change / leading projects or initiatives. Research / audit.
Skills and Knowledge Essential
Ability to provide and receive complex, sensitive or contentious information. Able to work without supervision. Competent in the use of IT such as Microsoft Office. Evidence of CPD. Knowledge of national health and social care policy. Knowledge of evidence based guidelines e.g. NICE guidelines, NPSA alerts, current nursing legislation. Excellent communication skills.

Leadership qualities. Ability to communicate effectively. Desire to work in Specialist Palliative care. Understanding of Specialist Palliative care Philosophy.
