Job summary Band 5 CAMHS Practitioner - Think Family Outreach Team The Think Family Outreach Team (TFOT) is part of an exciting pilot project designed to deliver an enhanced day provision for children, and community support for young people, and their families within the urgent care pathway. The post holder will work as part of the TFOT to support children, young people, and their families experiencing emotional distress. The aim is to provide early intervention to reduce the risk of frther escalation to Crisis and Home Treatment Teams (CRHTT) or acute hospital admission. The clinical offer is tailored to meet the individual needs of each young person and their family, with a strong focus on empowerment, resilience, and skill development. Key Responsibilities
Deliver clinical support as part of a multidisciplinary team. Provide a range of psychoeducational interventions to support crisis management and recovery. Work collaboratively with families to promote understanding, coping strategies, and emotional wellbeing. Contribute to care planning and ongoing assessment of needs within the crisis pathway. Support a strengths-based, family-focused approach to care.
Main duties of the job
There is a clear structure and career progression pathway within CYP Mental health with opportunities to broaden your experience and development , we are also part of the CYP IAPT initiative and work in partnership with the Reading CYP-IAPT collaborative where we have on going opportunities for further training.
You will hold a caseload of Children and Young People and have the opportunity to work alongside and liaise with other agencies in order to meet the needs of the young person and their family. You will also provide assessments under the supervision of team members of band 6 and above and be part of the ongoing multi agency/ multi-disciplinary discussion and support around the young people you will see.
The post holder will be expert in de-escalating crisis and supporting Children, young people and family/Carers in distress and an emotional state.
