Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.
You can help people in your community and start a great new career by becoming a Support Worker for the NHS and Kent County Council Home First Team.These are roles working across health and social care to provide support for people leaving hospital to help them returnhome safely and continue their recovery in their own surroundings.You'll help people to regain their confidence by helping with personal care, eating and drinking and staying as active asthey can.You'll have the full support of a large team of health and social care professionals at all times and your best reward willbe the smiles on the faces of the people you help to get home, where they belong.In addition to your qualifications, we'll give you all the training you need to provide a truly valuable service to people inyour community. So, when someone in hospital says "I just want to go home", you'll be there to help them do just that.

Main duties of the job
- Keeping the people who use our services as safe as possible through the use of sound judgement and effective riskassessments, escalating concerns to the relevant professional in a timely manner.- Enabling the person to be independent with their personal care, including washing, dressing and toileting.- Enabling the person to regain, where possible, their previous levels of independence taking their interests and previousskills, encouraging new skills.- Ensuring the people using our services have a good experience by respecting, empowering and working in partnership with people throughout the support planning process, respecting their wishes, choices and providing a person-centredapproach.- Support and advise people and their closer support network on how best to manage their condition in the context ofliving in their home, with a non-judgmental approach, whilst maintaining privacy, dignity and respect to ensure the higheststandards at all times.- Support a person in integrating/re-integrating into community-based activities.- Signposting and advising on longer term health and social care needs.- Give advice and guidance to access equipment /minor adaptations for a person's home.- Ensure all practice promotes equalities recognising anti-discriminatory behaviour, respecting confidentiality and individual rights and choices and also all practise is carried out within Health & Safety policies.
