Job summary
We are looking for an experienced Clinical Coder to join our busy GP practice, supporting accurate, high quality clinical data to underpin patient care - QOF - IIF and wider contractual requirements.
Main duties of the job
The postholder will be responsible for reviewing and coding clinical information from consultations, correspondence and results using SNOMED CT, ensuring data is complete, consistent and aligned with national business rules.
You should have strong attention to detail, good understanding of primary care clinical systems such as EMIS and Docman, and experience of clinical coding in a GP or NHS setting. Knowledge of QOF, Network Contract DES requirements and data quality principles is highly desirable. Excellent communication skills, a methodical approach and the ability to work independently to deadlines are essential.

The main responsibilities are:
a. Review all clinical letters, ensuring they are processed appropriately in line withextant protocols, from Emis document workflow, Docman inbox, Docman email,Accurx inbox and paper correspondence.
b. Accurately clinically code (SNOMED CT) data into the patient record on the clinicalsystem - Emis document workflow, Docman inbox, Docman email, Accurx inboxand paper correspondence.
c. Manage GP2GP allergy degrades and update clinical code.
d. Highlight all timely actions, changes to patient care and management to therelevant clinician
e. Identifying relevant clinical information documented in patient correspondence,coding appropriately and completing any detailed action
f. Inputting data as required to support practice reports, audits and to capitalise uponboth patient safety and QOF income
g. Scanning patient related documentation and attaching scanned documents topatients healthcare records
h. Review records and accurately produce a summary of the patients medical history

Person Specification
Qualifications Essential
A good standard of education with and expectation of having both GCSE Maths and English at Grade C (Level 4) or above, or Functional Skills Level 2 in Maths and English
Desirable
NVQ Level 2 in Health and Social Care or higher
