At Seaside Medical Centre, we are committed to delivering high-quality, patient-centred care to our diverse local community. We are seeking an enthusiastic and motivated Practice Nurse to join our friendly and supportive team.
The successful candidate will provide a wide range of nursing services, including chronic disease management, cervical screening, immunisations, wound care, health promotion and patient education. Working collaboratively with the wider multidisciplinary team, they will contribute to the delivery of safe, effective and evidence-based care.
We are looking for an individual who is passionate about patient care, able to work both independently and as part of a small nursing team, and who demonstrates initiative, adaptability and sound clinical judgement. Previous experience as a Practice Nurse in General Practice is desirable but not essential, and applications from nurses with transferable skills and an interest in primary care are welcomed.
This is a full-time position working 37.5 hours per week.
Main duties of the job
The successful candidate will play a key role in delivering high-quality nursing care and supporting the health and wellbeing of our patients. Responsibilities include:
Delivering patient-centred care in line with local and national guidelines and best practice.Administering vaccinations and immunisations, including routine childhood and adult programmes, travel vaccinations and new initiatives.Undertaking clinical procedures and diagnostic investigations, including venepuncture, ECGs, spirometry and peak flow measurements.Providing treatment and monitoring for patients requiring injectable therapies and long-term nursing interventions, including those with osteoporosis, cancer and haematological conditions.Supporting patients following new diagnoses by providing education, advice and guidance.Assessing and treating minor injuries and delivering evidence-based wound care, including the management of leg ulcers and complex dressings.Carrying out ear irrigation in line with current clinical guidance.Promoting healthy lifestyles and supporting behaviour change through patient education and personalised care planning.Working collaboratively with the multidisciplinary team to deliver safe, effective and coordinated care.Contributing to the ongoing development and improvement of nursing services within the practice.

The Practice Nurse will be responsible for delivering high-quality, patient-centred nursing care to the practice population, working both independently and collaboratively within the multidisciplinary team. The post holder will provide a range of clinical services, health promotion activities and chronic disease management in accordance with professional standards, practice policies and current evidence-based guidelines.
Key Responsibilities
Undertake nursing assessments and provide appropriate care, treatment and management for patients in line with their individual needs and own scope of practice.Deliver a range of practice nursing services, including chronic disease management, cervical screening, immunisations and vaccinations, wound care, ear irrigation, health promotion and lifestyle advice.Work collaboratively with GPs, Advanced Care Practitioners and other members of the multidisciplinary team to deliver safe, effective and compassionate patient care.Support the identification of practice population health needs and contribute to the development and delivery of services to meet those needs.Demonstrate and apply knowledge of the Quality and Outcomes Framework (QOF) and support the achievement of practice targets.Provide education, support and self-management advice to patients living with long-term conditions.Offer specialist advice and care to patients with respiratory conditions such as COPD and Asthma and where appropriately qualified, Diabetes.Undertake clinical procedures and investigations as required, within competence and training.Maintain accurate, contemporaneous clinical records in accordance with professional, legal and organisational requirements.Participate in clinical audits, quality improvement initiatives and service development activities.Engage in continuing professional development and undertake training required to support role progression and service needs.Practice at all times in accordance with the NMC Code, maintaining professional standards, accountability and patient confidentiality.
Person Specification
Qualifications Essential
Registered First Level Nurse with current NMC registration. Demonstrable commitment to professional development.
Desirable
Previous work experience within General Practice Mentoring/teaching qualification. Clinical supervision training and experience.
Experience Essential
Recent primary and community nursing experience. Involvement in implementing and using protocols and clinical guidelines. Experience of EMIS or similar clinical system.
Desirable
Management of long term conditions. Post registration experience. Leadership.
Knowledge and Skills Essential
Knowledge of needs of patients with long term conditions. Aware of accountability of own role and other roles in a nurse led service. Knowledge of health promotion strategies. Awareness of clinical governance issues in primary care. Knowledge of patient group directions and associated policy. Clinical skills - cervical cytology, immunisation and vaccination, wound healing, ear care and minor injury.
