About this role

Job summary

This Senior Clinical Assessor role will deliver a high-quality standard of clinical assessment, review and management of AACC cases, working collaboratively with key partners to achieve best outcomes. The post holder will work with the Clinical Manager to ensure that they deliver the highest quality and most appropriate services to patients and drive continuous service improvement.

Main duties of the job

Deliver a high-quality, person-centred Continuing Healthcare (CHC) assessment experience that is consistent, transparent, and equitable for all individuals and families.

Support smooth transitions between services and resource provision across all aspects of continuing care.

Ensure every assessment is completed to a consistent, high standard, regardless of who the person is, where they live, or when the assessment takes place.

Work as a Nurse/Allied Health Professional (AHP) Clinical Assessor within the Essex ICB Adult All-Age Continuing Care (AACC) team.

Implement the NHS Continuing Healthcare Framework for adults aged 18 and over across Essex.

Work under the guidance and support of CHC Clinical Managers.

Ensure accurate records management.

ROLE PURPOSE / SUMMARY

The Essex Integrated Care Board (ICB) is a new organisation formed through the consolidation of three ICB areas in response to the NHSs ambition to deliver a more streamlined, strategic and cost-effective model of health commissioning. The ICB will oversee the commissioning of care for the approximately 2 million people who live in Essex. Operating within a reduced running cost envelope and aligned to the new requirements of ICBs in England, the ICB will focus on improving population health outcomes, health equity and smarter resource allocation.

The AACC service delivers the statutory duties of the ICB to assess and to provide services that meet the identified needs of those found eligible for Continuing Healthcare. The service delivers that duty through the implementation of the National Framework for Continuing Healthcare and NHS Funded Nursing Care and the National Framework for Children and Young Peoples Continuing Care.

This Senior Clinical Assessor role will deliver a high-quality standard of clinical assessment, review and management of AACC cases, working collaboratively with key partners to achieve best outcomes. The post holder will work with the Clinical Manager to ensure that they deliver the highest quality and most appropriate services to patients and drive continuous service improvement.

As the ICB transitions to become a leaner, more focused strategic commissioning organisation the post holder will be expected to support the AACC in delivering on the three fundamental shifts set out in the NHS 10-year plan:

From Treatment to Prevention: Embedding preventative, personalised and population based approaches to commissioning.

From Hospital to Community: Championing care model redesign that supports community and neighbourhood health.

From Analogue to Digital: Enabling clinically driven transformation through digital innovation, data analytics and evidence based improvement.

KEY DUTIES AND RESPONSIBILITIES

To deliver on the job description set out above, the job holder will be expected to deliver on the requirements set out below:

Policy and service development

Support implementation of policy and or service developments related to AACCSupport team to ensure compliance with relevant ICB policiesUse research, innovation and audits to inform service improvements or performance management

Strategy and analysis

Understanding of complex qualitative and quantitative data relating to AACCHelps present insights and reports, draw from complex data, that can be used to inform decision making and deliveryUse of data and insight to contribute to reporting, including performance management, financial and quality reporting against agreed contractual or project deliverablesUnderstanding of Population Health Management methodologies and tools and how to use them to inform projects and programmes that will improve outcomes for patients and sustainability of health services

Planning and organisation

Support development and delivery of project plans relating to AACC, ensuring appropriate governance, risk management, escalations and reportingLead coordination of meetings and events to support delivery of project plansManage competing priorities, prioritizing own and team resources to effectively deliver on short, medium and longer term prioritiesEnsure accurate reporting and records managementSupport delivery and monitoring of business plan for AACC

Person Specification

Governance & Assurance Essential

Experience supporting delivery of projects that require collective decision-making and how to negotiate success in those situations A good understanding effective corporate governance and the route for decision making within the ICB

Qualifications Essential

Educated to Degree level (relevant focus) or equivalent managerial experience Evidence of sustained commitment to continuing professional development Registered professional qualification and currently registered with the relevant professional body
