Job summary

The role is for a Registered Nurse working in a small team assessing, evaluating and reviewing the health and care needs for people living with dementia and supporting their families. This will include Caring for peoples emotional, spiritual, physical and social health.

They will have a small caseload, support Band 3 Support Workers in their roles, help run Cognitive Stimulation Groups locally.

The RN will be essential to the wider MDT working in the local community and also liaising with the healthcare staff in the Royal Devon University Hospitals. Helping with preventing unnecessary lengthy hospital stays.

The Axe Valley Dementia team is a small dynamic and supportive team based in Axminster. The team visit and support people registered at Axminster, Seaton and Colyton and Townsend House Medical Practices with a diagnosis of dementia. We are looking to recruit a Band 5 Registered Nurse who is motivated and excited to work with people living with dementia and their families/carers over 3 days a week (22.5 hours).

Axe Valley Dementia service is part of the wider Multi-Disciplinary team in the area, specialising in the assessment, evaluation and the review of any ongoing health and care needs of people living with dementia. The team also facilitate Cognitive Stimulation Therapy and Meaningful Activity sessions for people living with dementia in the Axe Valley area, the RN will be overseeing the running of some of these sessions.

The teams whole intention is to keep people living with dementia active, independent and included in society, to prevent unnecessary hospital admissions, expedite safe discharges of people when they are in hospital, reduce the chances of carer breakdown and delay the need for 24-hour residential/ nursing care. The team prides itself on good care outcomes for each person with their care and needs.

The successful applicant will have ample opportunity to develop their skills in all aspects of dementia care. Training and support is available around delivering great dementia care. Having the ability to be a good communicator is an essential skill to have in this role. The applicant should have skills around observation of people, simple wound care and be organised. They will be expected to hold a caseload and work to supervise other staff to support the Community Dementia Matron.

A working knowledge of EPIC (My Care) would be an advantage.
