CCP specializes in providing comprehensive cardiology care and currently has over 30 locations throughout the Philadelphia region. We are currently hiring for a full-time Medical Assistant for our Transition of Care department to work at our Center City, Philadelphia, PA location. This position will be hybrid.
Please note this position is entitled to a $500 sign-on bonus.*
Responsibilities
• Coordinating patient care as they move between healthcare settings, including but not limited to:
• Post-discharge calls
• Scheduling follow-up appointments
• Medication reconciliation
• Patient education
• Connecting patients to resources to prevent readmission
• Working closely with providers and care teams on the best plan of action
• Route emergency calls appropriately and escalate when needed
• Perform general administrative support duties
• Other duties as assigned
Requirements
• Certified or Registered Medical Assistant preferred
• Must have at least 1-3 years of experience as a Medical Assistant
• Must have the ability to multitask
• Computer knowledge is a must
• Experience with an EMR system is a plus
• Must be well-spoken, friendly, and outgoing

*If you are hired, and assuming your employment is in good standing, you will receive the sign-on bonus. The bonus will be paid to you no later than 30 days following your start date. In the event that you voluntarily terminate your employment or the Company terminates your employment for cause within 6 months of your date of hire, you will be required to repay the full amount of your sign-on bonus.
