Job Description: The Specialist Social Worker provides comprehensive case management, care coordination, patient education, advocacy, and psychosocial support for older adults and their families. As a vital member of the Geriatric Medicine team, this role participates in comprehensive consultative appointments alongside medical providers, helping identify barriers to care, assess psychosocial needs, connect patients with community resources, and develop individualized care plans that support successful aging. The social worker also provides one-on-one patient appointments as clinically indicated and available.

Schedule: Monday - Thursday/32 hours

Key Responsibilities

• Provide case management and care coordination for patients and families requiring educational, social, emotional, and advocacy services. • Participate as a critical member of the interdisciplinary team during comprehensive geriatric consultative appointments, collaborating with medical staff to assess patient needs, develop recommendations, and support successful care planning. • Conduct individual patient and family appointments to address psychosocial concerns, care planning, resource navigation, and ongoing support. • Research, develop, and deliver educational materials and resources tailored to the learning styles, health literacy, and unique needs of the older adult population and their caregivers. • Assist patients and families in developing strong support systems by identifying needs and connecting them with appropriate community programs, agencies, and services. • Advocate for patients and families to help overcome barriers to care and improve access to needed services. • Work collaboratively with providers, clinical staff, payers, community organizations, and external partners to identify patient needs, strengthen community relationships, coordinate services, and enhance clinical outcomes. • Document assessments, interventions, referrals, and care coordination activities in accordance with organizational and regulatory standards. • Serve as a resource to the healthcare team regarding psychosocial issues impacting patient care and discharge planning.

The hourly pay range shown represents the base pay range for this position. Individual pay is determined based on factors such as experience, qualifications, skills, and market considerations. In addition to base pay, eligible team members may earn additional compensation through shift, weekend, holiday, on-call, and other applicable differentials.
