About this role

Job summary

Darwen Healthcare is an award winning, GMS Training Practice based in Darwen located off the M65, Junction 4. We are seeking to recruit a 6 session enthusiastic and motivated General Practitioner to provide quality medical services to our patient population of 14,800.

We host a range of Medical Students, Student Nurses, Physician Associates, Paramedics, Foundation Doctors and ST2/ST3 placements and participate in an extensive range of Enhanced Services i.e. Minor Surgery, IUCD, Implanon, Steroid Injections. Our Senior GP Partner offers a GPwSi service in Cardiology.

Clinical, nursing, palliative, frailty meetings and collaborative working within the Darwen PCN are key to providing quality and accessible care for patients.

This role is an ideal opportunity for developing specialist interests e.g. care of the elderly, frailty, palliative, Women's Health, Research or Medical Student Teaching.

The salary is dependent on experience, we offer flexibility and the start date is negotiable.

Main duties of the job

The post-holder will deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

Darwen Healthcare recognises the importance of Education and Training and as such you would be encouraged and supported to develop additional or increase existing skills that allow service development.

The post-holder will undertake a variety of duties e.g. surgery, video and telephone consultations, home visits,repeat prescriptions and repeat dispensing via EPS/ERD and administrative paperwork

The post-holder will complement our existing proactive team consisting of: 7 GP Partners, 4 Salaried GP's, and a diverse wider team consisting of ANP's, Paramedics, Pharmacist and a well established nursing team. Our Non-Clinical Team is led by a Practice Manager supported by Finance, Operations, Quality and Patient Engagement Leads and a large administrative team.

We are high QOF achievers, rated Outstanding by CQC and the practice utilises Emis Web, EPS/ERD, ICE, Iplato and AccuRx Messaging.

Clinical Responsibilities

In accordance with the practice timetable, as agreed, the post-holder will be available to undertake a variety of duties as required, including:

Surgery consultations will cover the breadth of the Practices core GMS and Enhanced Service obligations, which you will be expected to support the team in delivering

Additional or emergency presentations

Telephone and Video consultations

Visiting patients at home or at care providers premises

Share the daily responsibility of checking and e-signing prescription requests and dealing with queries via the Emis Web Clinical System. Increasing the uptake of the Electronic Prescribing Service (EPS) and Electronic Repeat Dispensing (ERD) where in the patients best interest

Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate, participating in medicines optimisation scheme as appropriate

Reviewing and following up results from requested tests, scans, investigations and referrals in a timely fashion. The practice ensures that all results are actioned daily.

Clinical paperwork and follow up as allocated via the Emis Web workflow management system in a timely fashion

Providing clinical supervision to the Clinical and Nursing Staff

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation

Assessment of the health care needs of patients with undifferentiated and undiagnosed problems

Screening patients for disease risk factors and early signs of illness

In consultation with patients and in line with current chronic disease management protocols ensure tasks allocated to the Nursing Team for Follow Up are completed

Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate

Recording clear, accurate and concise consultation notes to agreed standards that include clear patient management plans and correct coding

Collecting data for audit purposes and complete one clinical practice audit annually

In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care

Management and Administration

Work as part of the team to deliver core GMS services

Work as part of the team to contribute to maximum achievement in the Quality and Outcomes Framework (QOF) across all areas

Work as part of a team to deliver both Direct and Local Enhanced Services

Participate in practice and multidisciplinary team meetings including the Locality Palliative Meeting and Darwen PCN.

Liaison and work with voluntary agencies and multi-disciplinary teams

Participate in audit of clinical and patient services

Participate in further development of the service

Other responsibilities within the organisation

The post holder may be requested to work additional hours to cover holidays/sickness and will be expected to be flexible with regards to the agreed schedule of hours stated in the Job Plan in order to assure business continuity

Awareness of and compliance with all relevant practice policies/guidelines

A commitment to annual learning and audit to ensure evidence-based best practice. Ensure all mandatory training and training courses specific to your role are completed.

Person Specification

Qualifications Essential

Qualifications: Essential Fully trained GP with Vocational Training Certificate Full GMC registration with a licence to practise Included on the NHS England Medical Performers List, or eligible for inclusion if newly qualified Participation in annual medical appraisal and revalidation, where applicable Enhanced DBS check (with adult and child barred list checks where required) Full UK driving licence and access to transport to undertake home visits Right to work in the UK Demonstrate commitment to personal development Evidence of self-directed learning & development Knowledge of the NHS Long Term Plan and current primary care developmentsExperience of working in NHS primary care/general practice. Good clinical record keeping. Effective multidisciplinary team working. Knowledge of clinical governance and safeguarding. Excellent communication and consultation skills. Competence with GP clinical systems (EMIS Web/SystmOne as applicable).

Desirable

MRCGP (where applicable) Experience of EMIS Web, Lexacom and Accurx Interest in developing a specialist clinical area, for example frailty, care of older people, palliative care, women's health (LARC), dermatology or medical education Experience of service development and quality improvement initiatives Experience of clinical audit and/or research Teaching, training or supervision experience Minor surgery and/or joint injection skills LARC accreditation (implants and IUD/IUS fitting)
