About this role

Job summary

An exciting opportunity to join the Patient Flow Team.

The post holder will support the Admin team and the wider Patient Flow team to maintain accurate and up to date records as required.

To attend and minute CRFD (Clinically Ready for Discharge) meetings regarding patients and their discharge pathways.

To interact with the team, tending to admin duties such as scanning, ensuring paperwork is correctly uploaded to the Patient Note Recording System (PARIS).

To update and maintain the correct information on the utilised spreadsheets, where information is collated and used in meetings regarding patients discharge pathways

Main duties of the job

The post holder will:o Maintain a CRFD tracker using Microsoft Excel and ensure all this information is transferred onto the PARIS systemo Attend all relevant meetings relating to Flow - particularly but not exclusively the CRFD meetings and MADE meetingso Liaise with external providerso Support the admin team as requiredo Work closely with the Park House Patient Flow Team and Senior Patient Flow practitioners

Job description Job responsibilities

The post holder will: Maintain a CRFD tracker using Microsoft Excel and ensure all this information is transferred onto the PARIS system Attend all relevant meetings relating to Flow particularly but not exclusively the CRFD meetings and MADE meetings Liaise with external providers Support the admin team as required Work closely with the Park House Patient Flow Team and Senior Patient Flow practitionersThe post holder will: Communicate in a clear and respectful manner at all times. Communicate effectively with a range of individuals escalating any concerns. Receive and communicate all information received in a variety of forms. Provide and receive routine or complex information as required to support the Admin team and Head of operational support Develop excellent working relationships with all Trust staff and external providers.

The post holder will: Be expected to prioritise and organise their workload Work in a busy and pressured environment. Contribute to managing the workload of the team.

Person Specification

Qualifications Essential

NVQ level 2 (or equivalent) or demonstrable admin experience Maths and English CGSE - Grade C / 4

Desirable

IT qualification or equivalent experience Administration qualification

Experience Essential

Previous administration experience Good computer literacy (including use of Microsoft Office and Excel)

Desirable

Previous experience in caring/customer service role Must demonstrate ability to work without close supervision and using own initiative to significant level
