*Responsibilities*
* Assess patient conditions and develop tailored care plans to meet individual needs across various healthcare settings. * Administer medications accurately and safely, following established protocols and medical orders. * Utilize electronic health record systems such as Epic and Cerner to document patient information meticulously. * Perform clinical procedures including phlebotomy, wound care, and pain management interventions. * Collaborate with multidisciplinary teams to coordinate case management efforts and optimize patient outcomes. * Provide specialized care for pediatric patients, seniors with dementia or requiring assisted living support, and those needing home health services. * Educate patients and families on medical terminology, medication administration, and self-care techniques to promote recovery and well-being. * Maintain compliance with healthcare regulations and ensure safety standards are consistently met.
*Experience*
* Proven hospital experience with a strong background in acute care environments. * Prior experience working with pediatric populations and senior citizens, including dementia or Alzheimer?s care. * Familiarity with medical office procedures and laboratory processes such as phlebotomy. * Hands-on experience using electronic health records systems like Epic or Cerner is highly preferred. * Knowledge of pain management strategies and medication protocols for diverse patient needs. * Experience in case management or home health nursing is a plus, demonstrating versatility across settings. * Strong understanding of medical terminology combined with excellent patient care skills to communicate effectively with diverse populations.
