Job Description As a General Outpatient Nurse within our Certified Community Behavioral Health Clinic (CCBHC), you will provide comprehensive nursing care that supports whole-person wellness and recovery. This role combines clinical expertise, care coordination, patient education, medication management, and regulatory compliance to ensure seamless, high-quality care delivery. Working as part of an interdisciplinary team, you will support patients across multiple levels of care, facilitate transitions following hospitalization, coordinate treatment services, and promote positive health outcomes through evidence-based nursing practices. Direct Patient Care & Clinical Services Deliver office-based and community-based nursing services in accordance with treatment plans and provider orders. Provide patient-centered care that supports both physical and behavioral health needs. Conduct health assessments and identify barriers, risks, and opportunities for improved patient outcomes. Perform urine toxicology screenings, breathalyzer testing, and supervised screenings as required. Lead triage efforts for walk-in and telephone inquiries, ensuring timely and appropriate care. Medication Management & Care Coordination Administer medications safely and accurately while monitoring effectiveness and potential side effects. Complete medication reconciliation following hospitalizations and care transitions. Coordinate appointments and follow-up care for patients discharged from psychiatric, inpatient, detox, or partial hospitalization programs. Support medication-related processes, including prior authorizations and provider orders. Collaborate with providers, therapists, laboratories, and community partners to ensure continuity of care. Patient Education & Support Educate patients on chronic health conditions, behavioral health diagnoses, medications, treatment options, and wellness strategies. Promote preventive care and healthy lifestyle practices. Support treatment adherence by building trusting and professional relationships with patients. Documentation & Compliance Maintain accurate, timely, and complete clinical documentation within the electronic health record. Ensure compliance with organizational policies, regulatory requirements, and clinical standards. Review laboratory results and coordinate follow-up actions when needed. Maintain accurate records related to medications, screenings, care transitions, and follow-up plans. Team Collaboration & Program Support Participate in interdisciplinary team meetings, clinical reviews, and community partnerships. Assist with Meds Only program case management and closure processes. Conduct outreach and follow-up activities for emergency service calls and urgent patient needs. Contribute to a culture of quality, collaboration, and continuous improvement.
