About this role

Job summary

An opportunity has arisen for an experienced Admiral Nurse or experienced nurse, with an interest in dementia, within the Bradford District Care Trust team of Admiral Nurses, part of the award-winning Proactive Care Team (PACT).

An Admiral Nurse is a senior nursing position specialising in dementia care, relationship-centred support to individuals with dementia and their families, including complex assessments, clinical interventions, education for other healthcare professionals, and advocacy for quality dementia care within a designated area, all while adhering to the Admiral Nurse competency framework.

Main duties of the job

The primary focus of the Admiral Nurse role is to deliver person -centred and relationship- centred dementia care within Bradford to improve the experience of people living with dementia, their carers and families.

he post holder will:

o Deliver evidence based, culturally appropriate dementia care and support improvements in the experience of people living with dementia, their carers and families, applying the Admiral Nurse Competency Framework at a specialist level.

o Work with families to provide support, expert guidance and practical solutions

o Work closely with other services and organisations to co-ordinate care for people living with dementia and support for their families including planning for future care, access to care for co-morbidities and access to specialist palliative care.

o Provide support/education to colleagues, other health and social care professionals/providers in dementia care.

Person Specification

Experience Essential

Post-registration clinical experience of working with people with dementia and their carers/supporters in different settings

Desirable

Experience of palliative care and long term conditions

Training Essential

Formal knowledge of dementia/ older people acquired through clinical post-graduate training/ education

Qualifications Essential

Registered Nurse (RNMH/RGN/RNLD)

Desirable

Additional nursing qualification.

Knowledge Essential

Knowledge and understanding of South Asian cultures. Able to work sensitively and effectively with individuals, families and community organisations in localities with high population of South Asian origin, and in intercultural contexts.

Desirable

Fluent speaker of Urdu and / or Punjabi and / or Gujurati.
