Our Population Health Team sits within the Chief Medical and Neighbourhood Directorate and provides system leadership across prevention, health inequalities, population health management and learning and change. We work with colleagues across the NHS, local authorities including Public Health, primary care, voluntary organisations, employers and communities to tackle the causes of poor health, improve outcomes and reduce inequalities for more than two million people.
We're looking for an enthusiastic and motivated Population Health Officer to join our innovative Population Health Team within the Chief Medical and Neighbourhood Directorate at NHS Hampshire and Isle of Wight.
Main duties of the job
This is an exciting opportunity to play a key role in delivering our Health and Employment Integration programme, including the national WorkWell programme, helping people with health conditions stay in, return to, or access work. Working across the NHS, local authorities, employers, the voluntary sector and wider partners, you'll help turn ambitious population health strategy into practical action that improves lives and reduces health inequalities.
No two days are the same. You'll coordinate complex programmes, analyse and interpret data, work with a wide range of partners, support programme governance and help shape innovative approaches that address the wider determinants of health.
As a Population Health Officer, you will play a central role in supporting the delivery of our Health and Employment Integration programme, including the national WorkWell programme, helping to improve health outcomes, reduce health inequalities and support more people to stay well and in work.

Working as part of our Population Health Team, you will:
Coordinate and support delivery of complex population health and health and employment programmes across Hampshire and the Isle of Wight.
Work collaboratively with colleagues across the NHS, local authorities, employers, the voluntary and community sector, and other system partners to deliver integrated solutions.
Use data, evidence and population health intelligence to inform decision making, identify opportunities for improvement and support evaluation.
Develop project plans, prepare reports and presentations, monitor progress and support programme governance.
Organise and facilitate meetings, workshops and stakeholder engagement activities.
Help translate national policy and local priorities into practical action that improves outcomes for local communities.
Contribute to innovative approaches that address the wider determinants of health, including employment, economic inactivity and health inequalities.
As a Population Health Officer, you will support the delivery of ambitious programmes that improve population health, reduce health inequalities and strengthen the integration of health and employment across Hampshire and the Isle of Wight.
Working within our Population Health Team, you will play a key role in coordinating and delivering the Health and Employment Integration programme, including the national WorkWell programme, while also contributing to wider prevention, population health and health equity priorities.
Your responsibilities will include:
Supporting the planning, coordination and delivery of complex population health and health and employment programmes.
Working collaboratively with colleagues across the NHS, local authorities, employers, Jobcentre Plus, the voluntary and community sector and other partners to deliver integrated services.
Applying population health approaches and evidence to support service improvement, reduce unwarranted variation and address health inequalities.
Using data, insight and evidence to inform decision making, monitor progress and evaluate programme impact.
Supporting programme governance through the preparation of reports, presentations, risk logs, action trackers and performance updates.
Coordinating stakeholder meetings, workshops and engagement activities, building strong relationships across organisations and professional groups.
Contributing to the development of innovative approaches that address the wider determinants of health, including economic inactivity, employment and skills.
Supporting the implementation of national policy and local strategic priorities, helping to translate ambitious plans into practical delivery.
Working flexibly across a range of population health programmes, supporting colleagues and contributing to a culture of continuous improvement and learning.
This role offers a unique opportunity to work across organisational boundaries and gain experience of strategic programme delivery within an Integrated Care System. You'll work alongside experienced public health specialists and practitioners, clinicians, commissioners, analysts and programme leads, developing your expertise in population health, prevention and partnership working while helping to improve outcomes for local communities.

Qualifications and/ or experience Essential
Degree or equivalent experience in a relevant subject Experience of working in a healthcare, public sector or partnership environment, supporting delivery of population health, prevention or health inequalities activity. Knowledge of population health, prevention or health inequalities Ability to manage multiple priorities and organise workload xcellent communication and partnership working skills Ability to analyse and interpret data and information Experience of supporting projects or programmes IT skills including Microsoft Office and spreadsheets Commitment to reducing health inequalities Understanding of the NHS/Integrated Care Systems
Desirable
