We are looking for a Healthcare Support Worker to join our team to deliver Palliative & End of Life Care for patients within the community.
Our service works closely with community services within our Trust, charitable & voluntary services & care agencies to address a wide range of care/support to improve end of life care for patients, their families/carers.
You must be able to demonstrate an understanding of palliative care and possess the dedication & commitment to support & care for individuals, their families/carers. Specific training will be provided to support staff development & ensure a quality service delivery.
Shift pattern of 8am-4pm / 10am-6pm, with staff being expected to work flexibly in response to service needs.
Your base will be at Norton Canes Health Centre, Cannock, WS11 9SE and you will need to be able to adequately travel independently across the South, East and West of Staffordshire to meet the demands of the role. There will be a requirement to visit the main base at Bradwell Hospital, Staffordshire, ST5 7NJ

Main duties of the job
As a Healthcare Support Worker, you will be involved in the first point of contact in the care pathway and assist in the delivery of outcome-based goals and self-directed goal planning.
As part of this role, you will be expected to:
Undertake person centred support planning Deliver support plans and safeguard vulnerable people Contribute to outcome focused reviews Encourage peoples independence, choice, health & wellbeing Encourage individuals use of equipment and assistive technology Facilitate individuals transition to alternative services and support if needed Engage in organisational learning and development Provide excellent patient care & support Recording with Care - complete documentation and feedback
