Main duties of the job
As a Band 5 Registered Nurse, you will provide safe, effective, and compassionate nursing care to patients within our Intermediate Care Unit, supporting them through their rehabilitation, recovery, and reablement journey.
You will complete holistic assessments, develop and review personalised care plans, and work with patients to promote independence and achieve their individual goals.
You will deliver a high standard of clinical care, including medication administration, monitoring patient progress, recognising changes in clinical condition, and escalating concerns appropriately. You will support safe admissions and discharges, ensuring patients receive coordinated care throughout their stay.
Working collaboratively within the multidisciplinary team, you will contribute to a positive, recovery-focused environment, working closely with patients, families, carers, and colleagues to deliver person-centred care. You will advocate for patients, promote dignity, choice, and independence, and ensure their individual needs and preferences remain at the heart of everything we do.
You will play a key role in supporting patients with chronic disease management, promoting self-management where appropriate, monitoring long-term conditions, recognising changes in health status, and contributing to proactive care planning to optimise health outcomes and prevent avoidable deterioration or hospital admission.

Job description Job responsibilities
A dynamic dedicated nurse is required to compliment our current team of staff within our intermediate care bed base. Providing rehabilitation for both step down and step up patients you need to have a good all round skill set to support the needs of these patients.
Working as part of a dedicated integrated health and social care team you will develop a variety of skills. A foundation of basic nursing skills is required.
Duties and Responsibilities:
Ensuring a high standard of care with intermediate care bed based facility.
Liaises with the Intermediate Care Sister / Charge Nurse to ensure information in relation to patient care/changes to care plan and the continuity of care.
Provides mentoring, teaching and assessing to support staff.
Liaises with all health care professionals and other agencies to ensure effective service delivery in the intermediate care facility.
Clinical
Perform comprehensive assessment of patient nursing needs.
Assess, plan, implement and evaluate programmes of care according to changing health needs under the direction of the Intermediate Care Sister / Charge Nurse.
Maintain accurate records in accordance with NMC.
Attend a multi-disciplinary case conference as the named nurse if appropriate.
To communicate and collaborate with other members of the Integrated Intermediate Care Team, PHCT, voluntary and statutory organisations to support individual patient care.
To establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.
To competently perform clinical skills related to the post.
To be professionally responsible for delegated visits under the supervision of the Intermediate Care Sister / Charge Nurse
To work without direct supervision.
Managerial
To be responsible for work allocations of support staff.
To complete monthly supervisions for support staff.
To be responsible for organising and prioritising own caseload.
Clinical Governance
Support clinical supervision within the team.
Maintain professional portfolio that demonstrates evidence of on-going professional development.
Ensure that practice is evidence based through undertaking audit and critically appraising literature.
Maintain clinical standards through reflective practice and clinical supervision.
Adhere to the NMC Standards: - Professional Code of Conduct, Scope of Professional Practice, Administrations of Medicines, Records and Record Keeping.
Identify any limitations/gaps in professional or clinical knowledge.
Contribute to the development of and implementation of policies, procedures and guidelines relevant to own area of work.

Teaching and Learning
Act as a mentor for pre-registration nursing students.
Support the development of the nursing team by teaching and assessing members of the team including nursing assistants.
Undertake mandatory in-house training and other training that supports the role.
Access clinical supervision.
Undertake training that is specific to the post.
Teaching of patients / carers and other professionals to support patients to maintain their optimum level of independence.
Personal Issue
Participate in the Appraisal and PDP process.
To adhere to all Trust Policies.
Report sickness and other absences to the identified administrator.
To provide accurate, timely information to the identified administrator regarding annual leave, expenses and study leave.
Supply required activity data related to patient care.
