Job summary
We are looking for an experienced clinical pharmacist to join our busy practice. You will be helping to cover maternity leaveon a Fixed term contract and supporting the practice as part of Pilot initiative, covering 24hours per week. You will be working along-side our multi-disciplinary team to deliver the best healthcare service for our patients.
Main duties of the job
As a Pharmacist you will be responsible for working alongside GPs and other member of the clinical team dealing with patients'clinical medication reviews, including those with complex polypharmacy.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system; deal with acute prescription requests. Furthermore, they will promote systems for safer prescribing and provide expertise in clinical medicines advice.
About us
Our practice is based in the park hill area of Sheffield. We have 4 GP Partners and 2 Salaried GP's. Our nursing team consists of, 1 pharmacist, 2 Practice Nurses and 1 Health Care Assistants and care coordinator.
We are part of the GPA1 PCN which consists of the 5 local practices which share variety of services including a physiotherapist and Mental Health Team.

The clinical pharmacist will be responsible for
Reconcile the medications of patients whose care is transferred back to primary care in a timely and effective manner and ensure clinical records are updated. Liaise with patients and other providers to ensure patients receive appropriate medication on discharge.
Proactively engage with patients to help reduce potential readmission by preforming polypharmacy reviews.
Update patient medication records in line with incoming clinical correspondence
To respond to medication queries from patients and staff in a professional manner, referring to the appropriate clinician in accordance with protocols and professional boundaries.
Provide support to patients to maximise the benefit from their medication
Provide telephone support for patients with medication queries
Provide professional telephone and written advice and support to patients and their carers
To maintain full and complete records of all patient contacts using appropriate clinical templates and coding

Experience Essential
An understanding of the work of General Practice
Desirable
Experience with emis web clinical system Minimum of two years post-qualification experience An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare including clinical audit
Qualifications Essential
Professional registration with GPhC Evidence of continuing professional development Independent prescriber Right to work in UK
Desirable
completed the CPPE primary care pathway
