GENERAL DESCRIPTION

• Licensed professional authorized by law to dispense drugs and medicines prescribed by consultants or other registered medical personnel. • Responsible for coordinating and overseeing all pharmacy activities to ensure the proper provision and dispensing of medications. • Works collaboratively with other healthcare professionals to provide accurate drug information and treatment support, ensuring optimal patient care and compliance with professional and legal standards.

JOB DESCRIPTION

Main Responsibilities

• Supervise, manage, and operate all outpatient and inpatient pharmacy operations, including dispensing, medication supply, sales, and store management.

• Implement, install, and execute safe medication handling, storage, labelling, and distribution processes in compliance with GPP, MSQH, and hospital policies.

• Screen, check, analyze, and validate prescriptions for accuracy, appropriateness, legality, and completeness prior to dispensing.

• Perform, analyze, and process medication appropriateness reviews and medication reconciliation as required.

• Provide professional drug information and liaise with healthcare professionals regarding dosages, interactions, contraindications, and treatment regimens.

• Supervise, check, and execute extemporaneous preparation and dispensing activities in accordance with approved standard formulations.

• Participate in bedside dispensing activities at inpatient wards to ensure accurate medication delivery and patient understanding.

• Monitor, analyze, control, and operate inventory management processes, including purchasing, receiving, issuing, storage, and internal distribution.

• Identify, collate, analyze, and report stock shortages, excesses, expiries, and discrepancies, and produce corrective action plans.

• Ensure, manage, and reconcile Dangerous Drugs and Psychotropic Substances, including documentation, registers, security, and regulatory compliance.

• Implement, execute, and maintain the “5 Rights” of medication safety throughout dispensing and distribution processes.

• Supervise and execute outpatient and inpatient pharmacy operations, including dispensing, medication supply, sales, and store management.

• Screen, analyse, and validate prescriptions for legality, accuracy, appropriateness, dosage, interactions, contraindications, and completeness prior to dispensing.

• Perform extemporaneous preparation and dispensing in accordance with approved formulations and standard procedures.

• Participate in bedside dispensing and ensure correct medication delivery and patient understanding.

• Participate actively in clinical intervention programs such as the Antimicrobial Stewardship (AMS) Programme, therapeutic drug monitoring, total parenteral nutrition and medication reconciliation.

• Execute, check, and submit scheduled and ad-hoc stock counts, drug check, poison purchase orders, and controlled drug records.

• Supervise, guide, provide, and assess pharmacy technicians and store staff, including training, competency assessment, and performance evaluation.

• Prepare, check, and submit staff rosters, leave schedules, and manpower coverage plans to ensure uninterrupted pharmacy operations.

• Document, collate, analyze, and submit reports on medication incidents, near misses, sentinel events, and adverse drug reactions, and execute Root Cause Analysis (RCA) with corrective and preventive actions.

• Gather, collate, analyze, and produce operational data, statistics, and performance reports, and liaise with the Chief Pharmacist and relevant committees on medication safety and risk management.

• Liaise, support, and act on behalf of the Chief Pharmacist during their absence, including sourcing, processing, and procuring medications during emergencies or shortages, and perform any other duties as assigned.

OTHER RESPONSIBILITIES

• Perform any other responsibilities as assigned by immediate superior or hospital management.

JOB REQUIREMENTS Education:

• Bachelor Degree in Pharmacy/Masters in Clinical pharmacy.

• Registered with the Malaysian Board of Pharmacy.

• Possess Poison License A.

Knowledge and Experiences:

• Entry level and above practice in the hospital setting.

• Exposure to all field/ discipline in Pharmacy Practice.

• Able to communicate fluently and write in English and Bahasa Malaysia.
