1:1 Personal Care Nurse Position Summary We are seeking a compassionate, dependable, and skilled Personal Care Nurse to provide one-on-one nursing care for an individual in their home or other designated setting. The nurse will deliver personalized, high-quality care while promoting the client's safety, comfort, dignity, and independence. This role requires excellent clinical judgment, strong communication skills, and the ability to build a trusting relationship with the client and their family. Key Responsibilities
• Provide individualized nursing care according to the client's care plan and physician's orders.
• Assess, monitor, and document the client's physical and emotional condition.
• Administer medications, treatments, and other prescribed therapies safely and accurately.
• Monitor vital signs and identify changes in the client's condition.
• Assist with activities of daily living (ADLs), including mobility and feeding, as needed.
• Assist with mobility, transfers, and fall prevention while maintaining client safety.
• Manage medical equipment such as oxygen therapy, feeding tubes, tracheostomy care, ventilators, or catheters, if applicable.
• Maintain accurate and timely documentation of care provided.
• Communicate effectively with physicians, therapists, caregivers, and family members regarding changes in the client's condition.
• Respond appropriately to medical emergencies and implement emergency procedures when necessary.
• Maintain confidentiality and comply with HIPAA and all applicable healthcare regulations.
• Foster a supportive, respectful, and compassionate environment that promotes the client's quality of life.
Qualifications
• Current and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN/LVN) license in NYS.
Preferred Skills
• Experience providing one-on-one private duty nursing.
• Experience caring for pediatric, or medically complex patients.
• Knowledge of infection prevention and control practices.
• Compassionate bedside manner and commitment to patient-centered care.
• Strong organizational and time-management skills.
Physical Requirements
• Ability to lift, transfer, and reposition clients safely.
• Ability to stand, walk, bend, and perform physical tasks throughout the shift.
• Ability to respond quickly in emergency situations.
