Job summary <h2>Job Introduction</h2><p>As a Band 6 Team Sister, you will play a key role within our Community Nursing Service, providing both clinical and operational leadership to support patients with complex health needs within their own homes and local communities. This is an exciting opportunity for an experienced and motivated nurse who is passionate about delivering high-quality community care. You will manage a varied caseload, working with patients requiring support with wound care, long-term conditions, end of life care and complex clinical interventions. Using your clinical expertise and leadership skills, you will help ensure patients receive the right care, at the right time, from the right professional. You will work closely with the Senior Sister and wider multidisciplinary team, supporting colleagues, driving service improvements and helping to develop a culture of continuous learning and high-quality patient care.</p><h2>Package Description</h2><p>You will be joining a supportive and experienced community nursing team that is committed to delivering outstanding care closer to home.
