Job summary <p><strong>Job Introduction</strong></p><p>As a Band 4 Associate Practitioner, you will join our valued Garden City Team based at Gravesham Community Hospital. This is an exciting opportunity for an experienced Healthcare Assistant, Senior Healthcare Assistant or Associate Practitioner looking to progress their career within community nursing. Working alongside Community Nurses and the wider multidisciplinary team, you will play a key role in delivering high-quality clinical care to patients in their own homes and community settings. You will support patients with a range of healthcare needs, undertaking delegated clinical interventions and monitoring patient wellbeing while helping to deliver personalised, patient-centred care. This is a varied and rewarding role that offers autonomy, responsibility and the opportunity to further develop your clinical skills within a supportive team environment.</p><h2>Package Description</h2><p>You will be joining a friendly and supportive community nursing team that is passionate about delivering excellent care and helping patients remain safe and independent within their own homes.
