We are seeking a motivated and experienced Practice Nurse with general practice experience to join our team.
The role requires independent clinical decision-making, with support from GPs and the wider multidisciplinary team when required. The successful candidate will be adaptable, proactive, and committed to delivering safe, evidence-based care.
There will be a need to work collaboratively with the general practice team to meet the needs of the patients, ensuring that practice policies and procedures are followed.

Sleaford Medical Group is seeking a motivated and experienced Practice Nurse to lead and deliver high-quality patient care. Working within a well-established, patient-focused GP practice, the post holder will play a key role in delivering evidence-based care in line with QOF requirements and national guidelines.
The role requires clinical autonomy, with the ability to make independent decisions while working collaboratively with GPs and the wider multidisciplinary team. The successful candidate will be adaptable, forward-thinking, and committed to providing safe, effective, and patient-centred care to meet the needs of the local population.
Main Duties of the Role
Clinical Responsibilities
To work within clinical boundaries, demonstrating safe clinical decision-making, delivering high quality clinical care. To provide essential treatment duties such as BP checks, Long Term Condition Reviews, NHS Healthchecks, prescribed injections, BMI and lifestyle advice Provide holistic assessment of wounds and implement wound care including complex wound management in line with current evidence-based guidelines Offer advice about childhood and adult immunisations and ensure vaccines/injections are administered under patient group directives (PGDs) or individual prescriptions (PSDs) Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule Actively participate in seasonal and special projects as requested e.g. flu campaign Act as chaperone when required Vaccine / cold chain storage, monitoring and recording Undertake health promotion to help educate patients To maintain accurate patient records and enter onto the computer using agreed Read Codes To collect data and participate in regular Practice audit and research projects To contribute to the preparation of any Practice development plans To maintain own annual registration with the NMC and adhere to Code of Professional Conduct To maintain own continuous professional development with the support of the practice as required Attend and participate in staff and Practice meetings and clinical training sessions.

RGN Qualification with current NMC Registration Experience in Primary Care Experience in chronic disease management

Experience working in Primary Care Self motivated Adaptability planning and organising Performing under pressure Using initiative Flexible Confidentiality Experience of working within professional boundaries and recognising own limitations, with a commitment to ongoing development. Proven ability to communicate effectively with patients, carers, and colleagues, both verbally and in writing, and to work collaboratively within a multidisciplinary team. Experience of maintaining accurate and contemporaneous electronic clinical records. Working knowledge of, and adherence to, NHS and practice policies, including Data Protection, Confidentiality, Infection Prevention and Control, Safeguarding, and Health & Safety. Experience of following established procedures and protocols within a clinical setting.

Knowledge Essential
Use appropriate infection control procedures, maintaining work areas in own designated clinical room so that they are clean, safe and free from hazards, reporting any potential risks. Understand and apply the principles of the cold chain. Know the health and safety policies and procedures within the workplace, including fire procedures, maintaining accurate documentation, consent, sharps and needlestick injuries.
