About this role

Job summary

This role supports people to achieve better mental health and wellbeing, increasing independence and supporting recovery. It is a key part of plans to transform community mental health support and learning from this role will feed into that change. This role works closely with people in communities to reflect their strengths, interests, and ideas for what matters to them about good mental wellbeing. You will work with partners to strengthen links between CWCs, the communities they serve and the services that can best support them. The CWC service is a partnership delivered by Barca, Touchstone and Northpoint.

Main duties of the job

The role includes the following duties. These may change as the post evolves, however will be withing the level of responsibility for this role.

Person-centred support

Offer focused support to individuals and groups with the aim of building Knowledge, skills, confidence, and resilience in people, as well as encouraging people to access local activities and services

Use health coaching and motivational interviewing techniques, identifying barriers to people accessing services, and work with them to overcome these.

Work with individuals to co-produce a personalised support plan to address their health and wellbeing needs as required, based on their priorities, interests, values, and motivations. Help them to navigate access to local resources and services and inspire them to develop the skills to help restore and maintain emotional wellbeing. Give people time to tell their stories and focus on what matters to me, build trust, providing non-judgemental support, respecting diversity and lifestyle choices.

Take a holistic approach to support people to identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, employment circumstances and unemployment, loneliness, isolation and caring responsibilities, signposting to services where appropriate.

Challenge stigma attached to mental health, particularly within certain communities, and raise awareness of the importance for everyone to look after their mental wellbeing.

Respond to requests for support from a range of primary and secondary care partners including third sector colleagues.

Build and maintain strong relationships with healthcare professionals across primary and secondary care, as well as multidisciplinary teams from statutory and voluntary sectors, to deliver a coordinated, joined-up approach, facilitating timely referrals, effective communication, and seamless support, including mental health provision.

Ensure ongoing assessment and management of risks within an attitude of positive risk taking

Community-based support

Explore ways to innovate to get the best out of the role, and to identify possible new opportunities that could contribute to system change and strengthen the offer to people with mental health issues within the local Community.

Where appropriate, connect and signpost people to community groups, activities, and statutory services to meet their needs and ensure individuals are engaged with their local community.

Be proactive in reducing barriers to people engaging in activities i.e., where appropriate, arranging appointments and transport and accompanying them to activities or helping with orientation.

Assist with community resource mapping exercises and maintain an up-to-date database of community resources; map where there are gaps in provision and work with other voluntary sector organisations to develop resources where most needed.

Work in partnership with Primary Care Networks, and community services, local partner organisations and external stakeholders, acting as the glue to link people with relevant experts, local assets and interventions, while reflecting on, sharing and implementing best practice.

Represent the organisation professionally with external agencies, promoting its values, ethics, and person-centred approach while collaborating with Community Connectors across LCP areas to share ideas, best practice, and support service development.

Monitoring and reporting

Monitor and record outcomes for all those accessing the service through the use of agreed assessment tools and quality outcome measures.

Maintain systems to keep accurate records relating to the delivery and outcomes of the service.

Gather and collate statistical and other information and data as required, reporting on activity and outcomes in accordance with agreed timescales, ensuring effective qualitative and quantitative monitoring and evaluation of the service.

General support

Develop an awareness of local and national developments and best practice in this area of work and attend relevant conferences, meetings and training events as required

Seek to improve personal performance, outcomes, contribution, knowledge and skills.Be responsible for personal learning and development where appropriate and undertake training, both mandatory and optional, to increase knowledge, skills and awareness.Undertake any other duties as directed by your line manager, in line with the responsibilities of this post

The Community Mental Health Transformation Programme is a partnership of NHS organisations, community and voluntary sector, Adult Social Care and service users and people with lived experience coming together to transform how primary and community mental health services for people experiencing severe mental health issues are organised and delivered. We have a great opportunity to join Northpoint as a Community Wellbeing Connector (Mental Health), working as part of a partnership to transform community mental health support across Leeds within our Community Mental Health Services (CMHS) service.

You will join us on a full time, permanent basis (35 hours per week) with a salary of £32,597 per annum (NJC Scale 6, SCP 20).

You will be responsible for:

Providing person-centred support to people to achieve improved wellbeing, recovery and greater independence

Working alongside individuals to understand “what matters to me”, developing personalised support plans that reflect their strengths, interests, priorities and aspirations

Building strong relationships with local organisations and voluntary community sector partners, including primary care to connect people with activities, services and opportunities that improve their quality of life and to provide joined up support

Please note, this is an office-based role at The Small Mill, Leeds, due to the nature of the work and the need for regular in-person support.

What are we looking for in our ideal Community Wellbeing Connector?

We are looking for someone who is organised, reliable and enjoys working in a varied, people-facing role.

A relevant Level 3 qualification in Health and Social Care, Mental Health or a related subject (or equivalent experience in mental health support)

Experience of supporting people with mental health needs or complex lives within statutory, voluntary or community settings

Knowledge of voluntary, community and statutory services within Leeds and an understanding of the wider factors affecting health and wellbeing, and experience of partnership working, building effective relationships and working collaboratively across organisations

Experience of managing a varied workload or caseload and responding appropriately to changing priorities

An understanding of safeguarding, confidentiality and risk management

A relevant qualification at NVQ Level 3 (e.g., NVQ Level 3 in Mental Health and Social Care) or equivalent, and/or significant experience in mental health support work.
