The successful applicant will be confident in delivering a broad range of primary care nursing services and committed to providing high-quality, patient-centred care. You will be organised, compassionate and adaptable, with excellent clinical skills and the ability to work both independently and collaboratively as part of a multidisciplinary team.
We are seeking a nurse with experience in managing long-term conditions, delivering immunisation programmes, cervical cytology, wound care and health promotion. Candidates who are keen to further develop their skills within a supportive practice will also be considered, with opportunities for ongoing training and professional development.

Key Responsibilities
Deliver a broad range of nursing services including: Wound care and dressings Cervical cytology Health screening and NHS Health Checks Injections and medication administration Travel health advice and vaccinations Childhood immunisations Adult vaccination and immunisation programmes Assess, plan, deliver and evaluate evidence-based nursing care in accordance with current NICE guidance and national best practice. Support the management of patients with long-term conditions, including diabetes, asthma, COPD and hypertension, by undertaking chronic disease monitoring, patient reviews and delivering evidence-based care in line with national guidelines. Identify and support patients at risk of developing long-term conditions and promote preventative healthcare. Develop and deliver health promotion and wellbeing initiatives. Maintain accurate, timely and confidential patient records. Ensure the clinical environment is safe, organised and compliant with Care Quality Commission (CQC) standards and infection prevention and control requirements. Monitor and maintain clinical stock levels, including ordering and storage of supplies. Support the Practice in achieving Quality and Outcomes Framework (QOF) targets, Enhanced Services and other contractual requirements. Continue to develop clinical knowledge and competencies through ongoing education and training. Communicate effectively and compassionately with patients, carers and the wider multidisciplinary team, adapting communication methods to meet individual needs. Qualifications Essential
Essential Qualifications Registered Nurse with current NMC registration and no restrictions on practice.
Desirable
Desirable Skills and Experience Previous experience working as a Practice Nurse within General Practice. Competency in wound care, cervical cytology and immunisations. Experience managing long-term conditions, including diabetes, asthma, COPD and hypertension. Experience undertaking chronic disease monitoring and patient reviews. Knowledge of the Quality and Outcomes Framework QOF. Experience working within Care Quality Commission CQC standards and compliance. Experience using EMIS Web and Docman. Experience delivering health promotion and chronic disease management.
