Job summary

West Camden Primary Care Network is seeking a part time enthusiastic and committed Mental Health Nurse to join our vibrant and welcoming multi professional team, you would be initially covering for maternity leave with the opportunity to extend to a permanent position. You will be working within an integrated model of care supporting people 18+, presenting with mental health needs who require assessment and ongoing care and support within the context of the primary care clinical team of GPs, Physician Associates and Clinical Pharmacists, Practice Nurses and Social Prescribers, supported by the local Community Primary Care Mental Health Team including our local in-house psychiatrist.

As a PCN we have had the benefit of integrated mental health professionals for many years and want to build on this experience and expand the role to help manage the significant portion of patients presenting with mental as well as physical health needs. We believe in a holistic patient centred approach to care and are building our practice and PCN teams reflect this. You will be employed by the PCN directly and will work across 2 practice sites.

Main duties of the job

Face to face and telephone consultations for patients presenting with mental health issues and recording of these on the practice system.Ongoing support to patients, including help navigating the Mental Health Services and reviewing as necessaryOnward referral to appropriate services when indicatedJoin in practice weekly MDT Meeting's to discuss cases with the whole team.Comply with organizational policies, including safeguarding, confidentiality, lone working, and health and safety protocols.

About us

As a PCN we are located over 2 sites, about 1 mile apart. We provide GP services for about 37,000 patients, based over 2 sites. West Hampstead Medical Centre is a modern and purpose-built site that is well connected to both tubes, bus and rail networks. We are a training practice, with GP trainees and medical students from 2 big teaching hospitals as well as a nurse associate. Abbey Medical Centre is housed in a modern building with a similar profile of GPs, physician associates, pharmacists and both GP trainees and medical students.

As training practices we have a supportive and nurturing ethic experienced by all, from clinicians through to admin staff. We place emphasis on working as a team and allowing individual interests to develop and flourish. Our patients have access to a psychiatrist allocated to the practices in house to help with mental health problems. We also have very active patient participation groups that guide our decisions as practices.

The Mental Health Nurse will receive referrals in a number of ways:

By direct booking into a clinic slot as advised by a clinicianVia NHS Mail from clinicians at either practice.By mutual discussion within the team

Suitable referrals can include patients with Anxiety and Depression, PTSD, Personality Disorder (maybe waiting for an appointment with a tertiary service).

Undertake holistic assessments and co-produce a personalised plan with patients, identifying their support needs. Record all information on the practice clinical systems. These will be either face to face or as a telephone appointment.

Medication reviews will be an important part of assessments and ongoing reviews and we hope to recruit someone who has knowledge of this and is an independent prescriber or can work towards this.

Attend and feedback at locality and practice-based multidisciplinary team meetings as required.

Work with members of the integrated care teams to support a joined-up approach to provision where needed.

The Mental Health Nurse should be able to identify when there is a need for urgent action or for a step-up in care and know when to seek advice and support from the GPs and the wider team to discuss patient-related concerns, referring the patient back to other suitable health professionals or the core team/crisis team when required.

Experience of working in the NHS within Mental Health services The ability to actively listen, empathise with people and provide person-centred support in a non-judgemental way Experience of working with people from all backgrounds and communities, respecting lifestyles and diversity. Knowledge of IT systems

Desirable

Experience of working directly in a community mental health services ideally within primary care Experience of collaborative working and of building relationships across a variety of organisations. Knowledge of clinical systems used in the primary care setting such as EMIS/DOCMAN
