Job summary
The Ealing Hospital Discharge Hub has an exciting opportunity for an enthusiasticand motivated individual with care experience and experience within hospitaldischarge pathways, discharge planning and working within a multiple disciplinaryteam.Your main role will be to support P1 discharges from Ealing Hospital into thecommunity in a safe and timely manner, hold an active daily caseload of patientsand to ensure they are discharged within the agreed time frame and complete prechecklists for P1 patients being discharged.The successful post holder will work in partnership with colleagues in the acutesector, social services, Ealing Community Partners, ICB and other communityhealth services to ensure that appropriate patients from the acute setting areassessed and transferred safely to appropriate community-based services in atimely and seamless manner.
Main duties of the job
To support P1 discharges from Ealing Hospital into the community in asafe and timely manner, hold an active daily caseload of patients and toensure they are discharged within the agreed time frame and completepre-checklists for P1 patients being discharged.o To work as a core member of the acute based Ealing Hospital DischargeHub team to build and deliver the shared vision for a hospital dischargeservice.o Be responsible to optimize hospital bed utilization and reduce the length ofstay by ensuring the timely, safe transfer/discharge of pathway 1 patientsto the most appropriate and agreed environment.o To act as a pro-active community health service advocate, on behalf ofWest London NHS Trust, through extensive knowledge of relevantcommunity-based pathways within the area of operation and ability tosupport effective discharge planning.o Work in close partnership with hospital colleagues and external agenciessuch as Social Services, ICB, Housing Associations, GPs and voluntaryorganisations. This will involve attending meetings outside the Trust andsharing Care Management Plans with the ASC and other communityproviders.
