About this role

Job summary Working with a supportive district nursing team, you'll deliver high quality care to patients in their own homes so they can stay safe and independent. You'll be part of a multi-disciplinary team that shares information, plans care and adapts quickly to patient needs. This role is vital in helping people manage long-term conditions, recover at home and receive compassionate end of life care where needed.

Main duties of the job What you'll do:

Carry out holistic assessments and plan care that meets each person's needs Deliver clinical care such as wound care, medicines support and injections Work closely with colleagues and partner services to coordinate care Keep clear, accurate patient records and report any concerns Support patients and families to understand and manage their health What we're looking for: A registered nurse or due to qualify soon with relevant training Understanding of community nursing and person-centred care Confidence to work both independently and as part of a team Good communication skills, including sensitive conversations Basic IT skills and a willingness to learn and develop We welcome applications from people of all backgrounds and offer a supportive environment where everyone is respected, included and encouraged to thrive.

Qualifications Essential

Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration

Desirable

Post-registration qualifications Mentorship qualification

Knowledge Essential

Demonstrate an understanding of the role of a community nurse Knowledge of legislation impacting on service delivery Knowledge of the modernisation agenda Demonstrate an understanding of evidence based practice Knowledge of local initiatives Demonstrate an understanding of the MDT Basic IT skills Knowledge and skills in the assessment process

Desirable

Able to mentor students Evidence of professional development Have a working knowledge and understanding of First Visit Criteria

Experience Essential

Basic health and safety awareness Experience of working within a healthcare setting Experience of working with families and carers Experience of working in a team Experience of working with computerised recording systems

Desirable

Experience of multi-agency working Experience of working within a community setting To have recently worked within a nursing capacity To be able to provide education to patients, carers, staff and other health care professionals
