Job summary
An exciting opportunity has arisen for a Band 5 Neurology Nurse Practitioner to join the Neurology Specialist Nursing Team at Gloucestershire Hospitals NHS Foundation Trust.
The post holder will provide high-quality, evidence-based nursing care and specialist support to patients living with neurological conditions, including Multiple Sclerosis (MS), Motor Neurone Disease (MND), Epilepsy, and Parkinson's Disease (PD). Working collaboratively within the multidisciplinary team, they will assess, plan, implement and evaluate care across both hospital and community settings, supporting nurse-led and consultant-led services.

Main duties of the job
A key element of the role is providing a single point of contact for patients, carers and healthcare professionals through the Neurology Advice Line service. This service receives over 600 calls each month and plays a vital role in delivering timely clinical advice, symptom management support, care coordination, and escalation where appropriate. The post holder will contribute to ensuring patients have rapid access to specialist guidance, helping to prevent unnecessary admissions and improve patient experience.
The role includes managing advice line enquiries, supporting outpatient clinics, undertaking inpatient reviews, delivering patient education, and contributing to integrated care pathways to ensure safe, effective and patient-centred care. The successful candidate will also participate in service development, quality improvement initiatives, clinical audit, and the delivery of local and national standards for neurological care.

The post holder will:
- Assess, plan, implement and evaluate care for patients with neurological conditions.
- Deliver safe, effective and evidence-based nursing care.
- Support consultant-led and nurse-led outpatient clinics.
- Undertake patient reviews across inpatient, outpatient and community settings where required.
- Provide telephone advice and virtual consultations to patients and carers.
- Support integrated care pathways across acute and community services.
- Interpret and escalating blood results in accordance with agreed clinical pathways.
- Maintain accurate, timely and contemporaneous patient records.
- Educate patients and carers regarding diagnosis, treatment and ongoing management.
- Recognize deteriorating patients and escalate concerns appropriately.
- Act as a specialist resource for ward staff and other healthcare professionals.
- Participate in clinical audit, service evaluation and quality improvement projects.
- Maintain infection prevention and control standards at all times.

Person Specification
Qualifications and Training Essential
Registered Nurse (Adult) with current NMC registration. Appropriate post-registration clinical experience in a relevant speciality. Ability to assess, plan, implement and evaluate patient care. Experience of patient education and health promotion. Ability to work independently and as part of a multidisciplinary team.
Desirable
Experience in assisting a nurse-led clinics Experience of telephone advice services
Knowledge and Skills Essential
Excellent presentational, communication and interpersonal skills. Act as an expert resource in educating the wider multidisciplinary team in the safe and appropriate management of patients who present to the service on referral and facilitate ongoing education needs to allow discharge planning. Acts as a role model and are able to lead by example to ensure the Trust's values and behaviours are reinforced throughout their area of practice. Demonstrates ability to question and challenge nursing practice in a constructive way so that standards of patient care are continually evaluated and improved Understand legislation in relation to People with Learning Disabilities.

Clinical/Experience Essential
Experience (through placement or employment) of working in an acute clinical environment Extensive experience of working with people who have neurological conditions. Previous experience in a relevant speciality.

Co-ordinate and utilise skills to assess, plan, take appropriate action/intervention and evaluate care interventions for individuals with neurological conditions, in line with the National Service Framework (NSF) for long term conditions and the National Institute for Health and Care Excellence (NICE) guidelines.

Display a caring attitude to patients, their relatives, colleagues and themselves in a stressful environment Demonstrate the enhanced aptitude required to initiate, evaluate and modify a range of interventions. Supports the Band 6, Band 7 and ACP and act as a specialist clinical expert in direct contact care, able to make high level skills decisions, informed by advanced diagnostic, assessment and treatment skills. Work collaboratively with an appropriate range of MDT and interprofessional resources, developing, maintaining and evaluating links to manage risk and issues across organisations and settings.
