Main duties of the job
As a Senior Individual Assessment and Commissioning Nurse, you will play a vital role in relation to completion of robust care needs assessments to help determine eligibility for NHS funding. For those who meet the criteria for NHS funding, you will work in partnership to develop a personalised care support plan to meet their assessed needs which support safe delivery of a high-quality, person-centred package of care either in their own home or within a care setting.
Working with adults and young people transitioning into adult services, you will:
Lead on assessment and case management Commission safe, effective, and tailored care packages Ensure care is delivered in a timely, person-centred, and outcome-focused way Work collaboratively with patients, families, providers, and multidisciplinary teams Navigate and apply national policy, guidance, and legislation Support safe and timely hospital discharges and transitions of care Contribute to performance targets, quality standards, and service improvement Working pattern

You will:
Be a registered nurse (Adult, Mental Health, or Learning Disabilities) Have strong clinical assessment and decision-making skills Be confident working across complex and evolving care needs Demonstrate excellent communication and partnership working Be organised, proactive, and committed to delivering high-quality outcomes Ability to facilitate MDT meetings, produce robust reports capturing care needs (including utilisation of Microsoft Office for accurate documentation) Whether your background is in acute care, community, mental health or learning disabilities, we welcome your experience.

Person Specification
Knowledge Essential
Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making. Conversant with national and local guidance on NHS continuing health care and NHS Funded Care Strong understanding of the operational management and clinical role of the NHS Understanding of the patient experience agenda & its contribution to quality Knowledge & understanding of the current NHS agenda and of primary care Demonstrated capabilities to manage own workload effectively and make informed decisions working to tight and often changing timescales. Ability to analyse cases where material is conflicting and drawn from multiple sources. Previous experience of caseload management. Track record in clinical quality & performance activities. Experience of partnership working with other agencies.
Desirable
Understanding of the commissioning agenda including its scope, structure, methods of operation, processes and inter-relationships Understanding of Local Authority functions. A good understanding of personalised care, in particular personal health budgets
Qualifications Essential
Educated to degree level or equivalent level of qualification or experience in working at a similar level in specialist area Nursing qualification or equivalent, for example Occupational Health, Physiotherapist, Social Worker etc with current registration. Evidence of CPD
Skills Essential
High level of interpersonal skills; excellent communication skills with the ability to communicate effectively with a wide range of individuals. Capability to work across boundaries and foster close collaboration. Ability to build effective working relationships across a range of health care and other organisations. IT Skills, ability to use Microsoft Office programmes (Word, Outlook). Accurate data entry and text processing skills. Negotiation and conflict management skills and the ability to influence. Flexibility and the ability to handle a rapidly changing and ambiguous environment.
