The Lead Nurse provides clinical leadership, operational oversight, and professional development for the nursing and wider clinical support team across the organisation. They ensure safe, effective, and evidence based nursing care, support service delivery aligned with QOF, IIF, and local commissioning priorities, and contribute to strategic planning within practices and PCNs.
The role combines advanced clinical practice with managerial responsibilities, ensuring high standards of patient care, compliance with regulatory frameworks, and continuous quality improvement.
They will lead and manage the nursing workforce, working closely with practice management and lead GPs to deliver practice priorities whilst maintaining high standards of consistent nursing care for the practice(s) population.
Clinically, the focus of the role is the delivery of evidence-based practice. As an autonomous practitioner the Lead Nurse is responsible for care delivered,demonstrating critical thinking and skills in clinical decision making. Working collaboratively with the whole of the general practice team to meet the needs of patients, developing, supporing and actively delivering on relevant policies and procedures, and providing leadership and direction for the nursing team.
Main duties of the job
1 Clinical Leadership & Governance
Provide professional leadership to practice nurses, HCAs, phlebotomists, and other clinical support staff.
Ensure compliance with CQC standards, infection prevention and control (IPC), safeguarding, and clinical governance requirements.
Lead on development, implementation, and review of clinical policies, protocols, and standard operating procedures.
Oversee clinical audits, significant event reviews, and quality improvement initiatives.
Act as the clinical lead for chronic disease management pathways (e.g., diabetes, respiratory, cardiovascular)
Lead on national campaigns, relevant incentive schemes and initiatives practice(s) have committed to.
Lead the nursing team in the planning and implementation of local guidelines, protocols and standards, and of local projects or initiatives.
Work as an effective and responsible team leader, supporting others and exploring the mechanisms to develop new ways of working
Lead nursing team activities that create opportunities to improve patient care
Communicate nursing view to Management Team.
Delegate appropriately, adopting the principles of safe practice and assessment of competence of nurses.

2 Clinical Practice
Deliver highquality, evidencebased nursing care ssess, plan, develop, implement, provide and evaluate treatment programmes
Prioritise health problems, intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Manage and treat patients with Chronic Diseases in line with professional competence, working to national and local policies and practice needs.
Diagnose and treat minor illnesses.
Review medication for therapeutic effectiveness, in accordance with evidence based practice and national/practice protocols, and work with patients to support adherence to prescribed treatments
Support and manage health needs of women presenting for family planning and cervical cytology consultations
Implement and oversee vaccination and immunisation programmes for both adults and children including patients travelling abroad
Make professionally autonomous decisions and be accountable for safe, evidence based, cost effective, individualised patient care
Make appropriate referral to other members of the primary, community and secondary team. Signpost appropriately
Where the post holder is an independent prescriber ensure safe, effective and appropriate medications are prescribed as defined by current legislative framework and local and practice guidelines.
Work on the ground clinically with other nursing professionals to actively lead shoulder to shoulder on the ground
Promote a learning environment with a no blame culture
Lead the QOF strategy and delivery for LTC management, maximising income
Responsible for managing and maximising ICB, PHE and NHSE driven targets (Childhood Immunisations, adult vaccinations, Flu campaigns, cervical cytology programme, contraception & sexual health services, travel health advice & vaccinations)
Longterm condition reviews (e.g., asthma, COPD, diabetes, hypertension)
Professional leadership and expert clinical advice on all aspects of wound care
Support delivery of PCN services such as enhanced access, anticipatory care, and personalised care initiatives
Provide clinical advice to the wider team and act as a point of escalation for complex cases
3 Service Development & Performance
