About this role

Job summary

The Family Nutrition and Lifestyle Advisor plays a key role within the Choose 2 Change Specialist Children and Young People's Weight Management Service, providing holistic support to children, young people and their families throughout their journey.

The postholder will deliver personalised healthy lifestyle and nutrition support, behaviour change interventions, family education, community engagement activities and assist with the day-to-day administration of the service.

Working as part of a multidisciplinary team, the advisor will support families through one-to-one appointments, group sessions, school-based delivery, community events and digital consultations, whilst also contributing to referral management, data quality, service administration and continuous service improvement.

This is a flexible, multi-functional role designed to ensure families receive seamless support whilst maintaining efficient service delivery.

You will join a specialist team comprising of a GP, Head of Service, therapists, nutritionist, counselling specialists and administrators. You will be involved in contributing to MDT meetings, leading clinics and 1-1 appointments which can occur via telephone, digital platforms or face-to-face community venues across the footprint.

Main duties of the job

The role will provide one-to-one healthy lifestyle support to children, young people and families, delivering evidence-based nutrition education, parent and carer sessions, workshops and group programmes. Key responsibilities include developing personalised healthy lifestyle plans, using motivational interviewing and behaviour change techniques, monitoring progress, completing anthropometric measurements and escalating clinical or safeguarding concerns appropriately. The postholder will also provide nutrition advice and practical food education, represent the service in schools and community settings, deliver assemblies and workshops, build relationships with schools, GP practices and community organisations, and promote referrals into the service. In addition, the role will maintain accurate electronic records, support referrals, appointments, audits, case studies and service improvement projects, while contributing to the development of resources and promotional campaigns.

Family Support

Deliver one-to-one healthy lifestyle appointments with children, young people and families.

Deliver evidence-based nutrition education using approved service resources.

Deliver parent and carer support sessions and workshops to improve knowledge, confidence and practical skills in supporting healthy family lifestyles.

Deliver group sessions as part of a service development plan and use participant feedback to continuously improve programme deliver.

Support families to develop personalised healthy lifestyle plans.

Use motivational interviewing and behaviour change techniques.

Monitor progress and review goals.

Complete anthropometric measurements.

Escalate complex clinical concerns to Nutritionists, GP or Clinical Lead.

Identify safeguarding concerns and escalate appropriately.

Nutrition Support

Deliver nutrition advice within agreed competency framework.

Reinforce healthy eating guidance developed by the service.

Deliver practical food education.

Deliver follow-up appointments under agreed protocols.

Engagement & Community Delivery

Represent the service at schools.

Deliver assemblies and workshops.

Attend community engagement events.

Build relationships with schools, GP practices and community organisations.

Promote referrals into the service.

Support promotional campaigns.

Administration & Service Support

Support referral processing and booking appointments.

Contact families regarding appointments.

Maintain accurate electronic records.

Assist with audits.

Provide cover for administrative colleagues where appropriate.

Develop resources.

Support audits.

Contribute case studies.

Support service improvement projects.

Skills and Competencies Required

The successful candidate will demonstrate the following skills and competencies:

Behaviour Change: Experience of delivering person-centred behaviour change interventions, supporting children, young people and families to achieve sustainable healthy lifestyle improvements through evidence-based approaches such as motivational interviewing and SMART goal setting.

Communication: Excellent verbal and written communication skills, with the ability to adapt communication styles to engage children, young people, parents/carers, professionals and community partners effectively.

Relationship Building: Ability to build positive, trusting and professional relationships with families, colleagues, schools, healthcare professionals and wider stakeholders to support collaborative working and improve health outcomes.

Engagement and Facilitation: Confident in delivering engaging one-to-one appointments, group programmes, parent workshops and community sessions, creating an inclusive and supportive environment that encourages participation and behaviour change.

Organisation and Time Management: Excellent organisational skills with the ability to manage a varied workload, prioritise competing demands, meet deadlines and maintain accurate clinical and administrative records.

Administration and Digital Skills: Competent in using Microsoft Office and electronic patient record systems, with strong attention to detail when recording information, monitoring outcomes and completing administrative tasks.

Team Working: A collaborative team player who contributes positively within a multidisciplinary team, shares knowledge and supports colleagues to deliver high-quality, family-centred care.

Adaptability and Flexibility: Able to work across a variety of settings including community venues, schools, clinics and digital platforms, responding positively to changing service needs and priorities.

Health Promotion: Passionate about improving the health and wellbeing of children, young people and families, with an understanding of health inequalities and the importance of delivering accessible, inclusive and culturally sensitive services.

Person Specification

Qualifications Essential

Educated to degree level in nutrition and/or in exercise, psychology, public health, other health related field or demonstrable relevant equivalent experience

Desirable

Qualified to be able to become registered with the Association for Nutrition (AFN) as a Registered Nutritionist (RNutr) CIMSPA (previously REPS) qualified Level 2 Gym instructor/Group exercise award or equivalent Registration with CIMSPA (this can be obtained within 6 months of employment) Sports Coaching qualification Level 4 qualification in Behaviour Change, Public Health or Health Improvement.

Job Specific Requirements Essential

The ability to travel independently across the organisational footprint including Mansfield and Nottingham Work flexibly and adapt to suit service need including evening and occasional weekend work as required

Experience Essential

Experience of working with children, young people and families within a health, education, community or wellbeing setting. Experience of delivering one-to-one and/or group-based behaviour change or healthy lifestyle interventions. Experience of delivering health promotion or public health interventions within community settings. Experience of planning and facilitating engaging group sessions, workshops or educational activities. Experience of working with vulnerable families and tackling health inequalities. Experience of working within a multidisciplinary team. Experience of maintaining accurate records and using electronic systems. Previous experience of engaging with children, young people and families in either a paid or voluntary capacity.

Desirable

Experience delivering parent/carer education sessions. Experience working within school settings. Experience supporting service improvement projects.
