Job summary
Railway Medical Group are looking for a Practice Nurse to join our friendly and dynamic team in Blyth, Northumberland.
We are an expanding GP Practice with over 27,300 patients in South East Northumberland, and are dedicated to improving the health and wellbeing of our diverse patient community.
We are looking for a motivated team player who can demonstrate a high standard of clinical excellence and knowledge to provide a safe, effective and easily accessible range of health services.
We will be open to considering part time and full time hours.
PLEASE NOTE: An interview date has been set for 25th February
Main duties of the job
Chronic Disease Management to include: Diabetes, Asthma, COPD, Chronic Heart Disease, Hypertension, to develop and implement care plans for patients with Long term conditions, promoting self-care.

Job description Job responsibilities
To assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
Chronic Disease Management to include: Diabetes, Asthma, COPD, Chronic Heart Disease, Hypertension, to develop and implement care plans for patients with Long term conditions, promoting self-care.
Cytology, Childhood immunisations, NHS Travel vaccinations, phlebotomy, womens health, injections
To keep accurate clinical detailed health records on Emis.
Undertake mandatory and statutory training requirements.
