Job summary As a Band 6 Virtual Ward Team Sister/Charge Nurse, you will join our innovative Virtual Ward Service, helping patients receive high-quality care in the place they call home. Main ResponsibilitiesIn this role, you will manage a defined Virtual Ward caseload, providing assessment, planning, coordination and evaluation of care for patients with complex healthcare needs. You will work autonomously while collaborating closely with the wider multidisciplinary team to ensure patients receive timely and effective interventions. You will undertake comprehensive clinical assessments, develop personalised care plans and coordinate services to meet patient needs. You will be responsible for supporting patients with long-term conditions, frailty and admission avoidance pathways, ensuring care is tailored, proactive and responsive. Alongside your clinical responsibilities, you will provide leadership and support to nursing colleagues within the team. You will help develop staff through supervision, appraisals, mentorship and clinical support, while contributing to service improvement, audit activity and quality initiatives. Qualifications Essential
Registered Nurse with current NMC registration. Minimum of three years post-registration experience. Experience working within community and/or acute healthcare settings. Experience supervising or supporting other staff members.
