Job Overview
York Medical Group is seeking a compassionate, dedicated, and motivated Healthcare Assistant to join our team in a dual-role position. This role provides an opportunity to support both the Complex Care Team (CCT) and the wider Healthcare Assistant team on site, ensuring high-quality, person-centered care is delivered to patients.
The successful candidate will work collaboratively across both teams, providing clinical and practical support while promoting dignity, independence, and well being for all patients.
Please note that the role will require working across various sites.
Main duties of the job
CCT Support (12 hours per week)
Provide healthcare support as part of the Complex Care Team. Assist with delivering safe, effective, and patient-focused care within the community setting. Support patients with their individual care needs, following agreed care plans and clinical guidance. Work closely with colleagues and other healthcare professionals to ensure continuity of care. Conduct lone home visits to patients within the community as required. Applicants must hold a full, valid driving licence, have access to a vehicle, and be willing to travel independently between patient visits and practice sites.
HCA Team Support (24 hours per week)
Provide high-quality healthcare assistance to patients within the on-site HCA team. Support clinicians during consultations, treatments, and procedures where required. Carry out routine clinical tasks within the scope of the role and appropriate training. Monitor and report changes in patients conditions or well being. Maintain accurate patient records and follow all relevant policies, procedures, and safeguarding requirements. Promote a safe, respectful, and welcoming environment for patients and colleagues.

Complex Care Team (12 hours per week)
Provide healthcare support to patients receiving care through the Complex Care Team.
Deliver safe, compassionate and person-centred care in patients homes and community settings.
Undertake delegated clinical and non-clinical tasks within the scope of competence and training.
Support patients with personal care and monitoring where appropriate.
Assist with the implementation of agreed care plans and report any concerns or deterioration promptly.
Carry out observations and clinical monitoring as directed.
Escalate concerns promptly to the appropriate clinician.
Maintain accurate and timely clinical records.
Work closely with GPs, nurses, pharmacists, social care professionals and other members of the multidisciplinary team.
Participate in case discussions and team meetings where appropriate.
Conduct lone home visits safely and professionally following the practice Lone Working Policy.
Travel independently between patients homes and practice sites as required.
Healthcare Assistant Team (24 hours per week)
Deliver a high standard of healthcare support within GP practice clinics.
Undertake clinical procedures appropriate to competency, including:
o Blood pressure monitoring
o Height, weight and BMI measurements
o Venepuncture (where trained)
o ECG recording
o NHS Health Checks
o New patient health checks
o Lifestyle advice and health promotion
o Urinalysis
o Simple wound care and dressings (where trained)
Prepare patients and equipment for clinical procedures.
Assist GPs and Practice Nurses during clinical procedures when required.
Maintain treatment rooms and ensure clinical equipment is clean, stocked and ready for use.
Follow infection prevention and control procedures at all times.
Report concerns regarding patient safety or wellbeing promptly.
Record clinical activity accurately within the patient record.

Person Specification
Experience Essential
Experience working as a Healthcare Assistant or similar patient-facing healthcare role. Experience working within a multidisciplinary team. Experience delivering patient-centred care.
Qualifications Essential
NVQ Level 2 or Level 3 in Health and Social Care (or equivalent), or willingness to work towards. Care Certificate or willingness to complete.
Other Requirements Essential
Full UK driving licence. Access to a vehicle insured for business use. Willingness to undertake lone home visits. Flexibility to work across York Medical Group sites. Commitment to ongoing learning and development.
Desirable
Experience within General Practice. Venepuncture competency. ECG competency. NHS Health Check experience. Experience using SystmOne. Experience of community-based working.
Knowledge and Skills Essential
