Job summary
We are looking to recruit cheerful, enthusiastic experienced Registered General Nurse to join our dynamic, experienced team.
We would like you to provide general nursing services including treatment, screening and advice according to policy and protocols and within the professional competence of the post holder.
The post holder will always work within the Code of Conduct as set out by the Nursing and Midwifery Council.
You must have experience of managing long term medical conditions, leading and working with multidisciplinary teams within the wider health and social care community and have a good understanding of the needs of the local population.
You will be actively involved and lead local health promotion campaigns and quality improvement initiatives in line with Primary Care Networks and General Practice.
You will work closely with wider teams to support improved patient outcomes.
Main duties of the job
Long term conditions management -assess, plan and evaluate individual treatment plans
Assessing care and nursing needs for patients in their own homes
Management of frail patients
Assess and evaluate treatment programmes that promote health and wellbeing
Support anticipatory care planning and advanced care planning discussions
Work with other health care professionals to diagnose, monitor, manage and treat long term conditions, including holistic and non drug based treatment methods
Wound management including compression therapy
Phlebotomy
Health promotion
Health screening
Undertaking planned and opportunistic tests and investigations at the request of other members of the clinical team
Assist in and perform routine tasks related to patient care as directed.
Hypertension management.
Weight monitoring and support.
Health promotion.
First aid and management of emergencies e.g. cardiac arrest, shock, haemorrhage.

Main duties and key responsibilities:
Maintain patient confidentiality and work within guidelines and protocols at all time
Initiate and arrange a methodical assessment of patient and service need
To be aware of health and safety issues affecting staff, patients and visitors and keep up to date with health and safety training
Maintain accurate records of the care given, ensuring accurate data input into computerised systems.
Assist and chaperone, when necessary, with the examination and treatment of patients.
Work collaboratively across the PCN to enhance integration with community, secondary and social care services
Be flexible to work to current PCN and General Practice priorities
Take a preventative approach to patient care
Work in the community and across PCN practices as necessary, including sometimes in nursing or care homes
Team working
Understand own role and scope, and identify how this may develop over time
Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working
Delegate clearly and appropriately, adopting the principles of safe practice and assessing competence
Ensure clear referral mechanisms are in place to meet patient need

Works safely and effectively when working autonomously and in conjunction with other health care professionals.
Desirable
