Job summary
We have an exciting opportunity for a Occupational Therapist at our Primary Care Network (PCN),The One Network Alliance Ltd is a Primary Care Network (PCN) covering the Bethnal Green locality. PCNs bring together neighbouring GP practices and partner organisations to deliver joined-up, high-quality care for their registered populations.
The post holder will work in partnership with service users to develop personalised recovery and support plans. The role will primarily support patients on the complex care caseload, many of whom are living with significant frailty, multiple long-term conditions and may be housebound. The focus will be on assessing individuals needs and identifying appropriate interventions to maintain independence and improve quality of life.
The post holder will work across the PCN footprint, delivering sessions from member practices and undertaking home visits where required. They will represent and promote the Occupational Therapy service across the PCN, working collaboratively with practice and wider PCN teams while also managing their work confidently and autonomously.
The post holder will work closely with, and report to, the Senior Occupational Therapist.
This is a part time role - 22.5 hours per week.

Clinical
To apply a high level of understanding of the effects of disability on occupational performance and provide training and advice on lifestyles changes and adaptations to the patients social and physical environment To have a high level of understanding of the effects of dementia and to undertake appropriate assessments and interventions helping those with dementia to retain existing function and to be able to safely live within their own home for as long as possible To assess patients capacity, gain valid informed consent to treatment and where such capacity is lacking/absent to work within Mental Capacity Act and associated legal frameworks in the management of the patient To promote social inclusion and physical and mental well-being using evidence- based practice. This is underpinned by the principles of hope and recovery, health promotion and relapse prevention To independently manage a clinical caseload of service user with complex health needs, demonstrating clinical professional judgement and liaising closely with team members where relevant Deliver proactive, holistic occupational therapy for individuals with frailty or rising need, supporting early intervention and prevention, hospital admission avoidance and maintenance of independence in line with neighbourhood priorities. To work proactively with people identified through population health approaches, multidisciplinary team meetings and risk stratification processes to prevent deterioration and maintain independence Collaborate as part of an integrated, multidisciplinary team, addressing the impact of physical, social, psychological, environmental, and mental health issues on functional independence To consider both physical and mental health needs as part of the occupational therapy assessment To plan and implement specialist individual and/or group interventions collaboratively, to enable service users to achieve valued goals and outcomes To identify any significant risk factors in line with local policies, with support from practice staff To identify and respond appropriately to safeguarding concerns relating to adults and children in accordance with local safeguarding policies and procedures To contribute to and work within clinical pathways and protocols To evaluate effectiveness of therapy interventions and quality of clinical decisions in conjunction with service users/carers, other professionals etc To assess the impact of the home environment on functional independence and provide recommendations regarding equipment, adaptations and support services where appropriate
Communication
