You will undertake clinical observations, support rehabilitation treatments and assist with assessments under the guidance of qualified practitioners. You will also play an important role in monitoring patient progress, providing feedback to the wider team and helping ensure patients receive coordinated, person-centred care. A key aspect of the role involves supporting pulmonary rehabilitation services, including helping to facilitate group rehabilitation sessions and encouraging patients to actively participate in their treatment and recovery. Key responsibilities include:
Supporting patients with rehabilitation programmes and treatment plans Undertaking clinical observations including blood pressure, temperature, oxygen saturations, respiratory rate and heart rate Supporting assessments and interventions under the supervision of registered clinicians Assisting with pulmonary rehabilitation groups and community-based rehabilitation programmes Monitoring and recording patient progress and reporting concerns appropriately Maintaining accurate and timely clinical documentation Working collaboratively with the multidisciplinary team to achieve positive patient outcomes Supporting service development and continuous improvement initiatives Acting as a key point of contact for patients, families and partner organisations where appropriateDue to the community nature of this role, a full UK driving licence and access to a vehicle is essential.
Person SpecificationEssential NVQ Level 3 in Health and Social Care or equivalent level 3 qualification Previous experience working as a Band 3 Rehabilitation Assistant, Therapy Assistant or similar role Experience working within a community rehabilitation or healthcare setting Ability to work independently as well as part of a multidisciplinary team Excellent communication and interpersonal skills Strong organisational and time management skills Ability to assess needs, implement care and prioritise workload effectively Understanding of confidentiality, information governance and person-centred care Confidence using IT systems and electronic records Full UK driving licence and access to a vehicleDesirable Experience facilitating rehabilitation or exercise groups Community nursing or rehabilitation experience Trusted Assessor Training Qualification relevant to rehabilitation, therapy or healthcare Foundation Degree in a relevant subject or willingness to work towards oneAbout the CompanyWe change lives by transforming health and care.
