Join our Out of Hours Service, where you'll play a vital role in delivering urgent care and supporting people with long-term conditions across Bristol, North Somerset and South Gloucestershire--helping patients stay safe, well, and independent in their own homes.
You'll play a key role in supporting patients on the Out of Hours caseload, delivering a range of delegated clinical and therapy-led interventions in community settings. This role offers a valuable opportunity to develop your Assistant Practitioner/Nurse Associate skills within a supportive, flexible, and experienced team.
Our service worksbetween the hours of 19.30 - 8:00.
Main duties of the job
Deliver clinical care delegated by the senior healthcare professional, in accordance with an agreed person centred plan of care. To undertake and complete the appropriate training, supported by a robust competencyframework, within the time allocated, in order to carry out the requirements of the role anddeliver a competent clinical service. Continuous maintenance of competencies following policy and avoiding gaps in service throughlapsed competencies Work within your level of competence, only performing those tasks you have been trained to doand feel competent to carry out. Deliver clinical care in the patient's own home within the scope of the Assistant Practitioner/Nurse Associateremit and in accordance with instructions and training received.

Qualifications & Training Essential
Full driving licence Relevant Assistant Practitioner course/Foundation Degree modules / Diploma in Clinical Health and Social Care / diploma level 3 or equivalent
Skills, Knowledge, Experience Essential
Experience of direct client care in a health care setting demonstrating a wide range of clinical skills Able to work without direct supervision Ability to work effectively as part of a team and build Able to communicate clearly, succinctly and sensitively using a range of means
Desirable
Experience of training and supporting others Recent relevant experience within the community setting
