About this role

Job summary

We are looking to expand our HCA team with an additional 18 hours part time role. This would be to work fully day Tuesday, Full day Wednesday and the afternoon on Thursday (room availability so not negotiable)

The HCA works within the organisation and supports the practice nursing and administrative teams in the delivery of clinical and administrative services. This role involves working as part of the practice multi-disciplinary team, delivering care within their scope of practice to the entitled patient population.

Having completed the Care Certificate or equivalent qualification, the HCA will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion, long-term condition management and patient education while also supporting the administrative team on an as-required basis.

The HCA will work under the supervision of a registered clinician while developing competencies in community-based care. The role is at Support Worker Level and as described in NHS E Primary Care and General Practice Nursing Career and Core Capabilities Framework

The post-holder will be an integral part of the general practice team.

Main duties of the job

Consult with patients and their families to undertake assessments of patient need

Deliver and assist clinical staff in the provision of treatment, preventative care, health promotion, long-term condition management and patient education

Be an integral part of the general practice team

Provide clinical support to registered nurses and other healthcare professionals within the organisation

Deliver basic clinical care interventions under supervision, supporting continuity of care and patient safety initiatives

Ensure specimens are recorded and ready for onward transportation

Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the organisations IPC Policy

Work within national and local protocols where these exist

Be aware of your duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children

Support in the delivery of enhanced services and other service requirements

Undertake all mandatory training and induction programmes

Maintain a clean, tidy, effective working area at all times

Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed

Job description Job responsibilities

he following are the core responsibilities of the healthcare assistant/ phlebotomist. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels and your experience:

a. Undertake new patient health checks, including blood/ height weight, alcohol and exercise screening.

b. Support the practice nurse with health promotion

c. Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, recording findings accurately

d. Facilitate routine and 24-hour BP monitoring, advising patients accordingly

e. Support the practice nurse with the management of chronic disease clinics

f. Carry out BMI checks as directed

g. Act as a chaperone as required

h. When trained, undertake venepuncture

i. Ensure specimens are recorded and ready for onward transportation

j. Provide support during minor operations as required

k. Ensure all clinical rooms are adequately stocked and prepared for each session

l. Ensure fridges are cleaned routinely in accordance with extant guidance

m. Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the practice IPC policies.

n. Deliver opportunistic health promotion where appropriate
