Job summary
An exciting opportunity has arisen for an enthusiastic and motivated nurse to join our established multidisciplinary Heart Failure Team as a Heart Failure Specialist Nurse on a 12-month fixed-term basis.
We are seeking an experienced cardiology nurse with a passion for delivering high-quality, patient-centred care. The successful candidate will possess excellent communication and interpersonal skills, alongside strong clinical expertise and qualifications relevant to the role.
Applicants should be able to demonstrate the ability to work independently whilst contributing effectively within a multidisciplinary team, supporting patients with heart failure across a range of care settings.
Applications are encouraged from experienced Band 7 nurses. However, exceptional Band 6 candidates with relevant cardiology experience, a commitment to professional development, and a keen interest in specialist heart failure care will also be considered. This offers an excellent opportunity to develop advanced knowledge and skills within a supportive and evolving specialist service.
Main duties of the job
We are seeking a highly motivated and autonomous practitioner to join our specialist Heart Failure Team. The successful candidate will play a pivotal role in optimising medical therapy and delivering comprehensive education and support to patients, relatives, and carers. Through proactive management and evidence-based practice, you will contribute to reducing unnecessary hospital admissions and readmissions while maximising patients' quality of life.
The role includes responsibility for contributing to data collection for the National Heart Failure Audit, as well as supporting ward and clinical staff in the development and implementation of heart failure management and discharge plans. Previous experience within specialist heart failure services is desirable, together with demonstrable clinical competencies relevant to the role.
You will become part of a dynamic, dedicated, and continually evolving multidisciplinary team, benefiting from a structured supernumerary induction period and ongoing professional support. The post will primarily be based within the South Tees locality; however, ongoing service reviews across the University Tees area may influence future models of service delivery and working practices.

Qualifications & Training (Band 6) Essential
Registered nurse (level one). Current Nursing and Midwifery Council Registration Evidence of continuing professional development.
Desirable
Specialist knowledge, underpinned by theory and experience. Professional knowledge acquired supplemented by post graduate diploma specialist training, experience, short courses plus further specialist training to degree equivalent level. To have completed the physical assessment and disease management skills course Heart Failure course Secondary Prevention in Coronary Heart Disease course Leadership qualification. A teaching qualification. ILS
Experience (Band 6) Essential
Relevant cardiology nursing experience Experience of managing patients with a long-term condition Can provide evidence of effective leadership and developmental work undertaken at senior level. Experience of working collaboratively in multidisciplinary teams

Knowledge & Skills (Band 7) Essential
Extensive post registration experience within cardiology and/or heart failure. Advanced knowledge of heart failure pathophysiology, investigation and evidence-based management. Knowledge of NICE, ESC and national heart failure guidance. Excellent clinical assessment and decision-making skills. Ability to interpret ECGs, blood results, echocardiography reports and other cardiovascular investigations. Excellent communication and interpersonal skills. Excellent organisational and prioritisation skills. Ability to work autonomously whilst recognising limitations. Excellent IT and electronic patient record skills.
Qualifications & Training (Band 7) Essential
Registered nurse (Adult) with current NMC registration. Degree in nursing or equivalent experience. Evidence of continuing professional development. Mentorship/practice assessor qualification (or willingness to undertake) or an alternative teaching qualification.
Experience (Band 7) Essential
Significant cardiology nursing experience. Experience managing patients with complex cardiovascular disease. Experience working within multi-disciplinary teams. Experience providing patient education and self-management support. Experience contributing to audit or quality improvement. Experience supporting service development. Experience supervising junior staff and students.
