Job summary
This post is a 10-month fixed term maternity leave cover position starting in October 2026.

You will be Advanced Clinical Pharmacist for the Acute Care wards and will support development the clinical pharmacy service to wards within acute care, including the Medical and Surgical Admissions Units, Short Stay Unit, Emergency Department and the Same Day Emergency Care Unit in line with local and national standards and policies, ensuring the safe, clinical and cost effective use of medicines and staff resources.
You will provide a highly specialised level of pharmaceutical care to patients and will support other healthcare staff in meeting risk management, financial management and clinical governance responsibilities around the use of medicines.
You will provide highly specialised advice for pharmacists and healthcare professionals, including participation in multidisciplinary and governance meetings.
You will contribute to the quality training experience and support for trainee pharmacists, pharmacy students and trainees in the multidisciplinary team, delivering training and supervision for junior pharmacists and medicines management pharmacy technicians.
You will carry out clinical pharmacy audit and practice research supported by the Clinical Pharmacy Services Manager.
Main duties of the job
Lead and coordinate the clinical pharmacy service in Acute Care areas, by providing a highly specialised level of pharmaceutical care. This will include visiting wards, speaking to individual patients to discuss all aspects of their medicines, ensuring that prescriptions are appropriate and safe, arranging supply, and when necessary, liaising with other healthcare professionals with regard to all aspects of that patients prescription and pharmaceutical care.
Practise as a highly specialised clinical pharmacist within local and national policies and standards to deliver medicines optimisation to individual patient's out a daily ward round to clinically review individual patients' treatment and to determine the most appropriate choice of medication in response to complex series of data relating to their condition, their response to treatment and reduction of avoidable adverse events.
Work with medical, nursing and the site management team to facilitate patient discharges in order to maximise patient flow.

Qualifications/training and professional development Essential
Master's degree in Pharmacy plus pre-registration training resulting in registration with the General Pharmaceutical Council as a Pharmacist Postgraduate Diploma in Clinical Pharmacy qualification or proven level of equivalent experience Independent Prescribing qualification, or working towards and will have completed by commencement in post
Desirable
Accredited pre-registration pharmacy student tutor and/or accredited clinical pharmacy diploma tutor MSc in Clinical or Technical Pharmacy Member of the Royal College of Pharmacy
Experience Essential
Extensive post-registration experience, with substantial hospital experience Extensive experience providing a pharmacy clinical service to patients Emergency and Acute Care Specialties Previous experience of supervising/mentoring junior colleagues Experience of implementing and/or reviewing policies, guidelines, PGDs or equivalent
