Job summary Assess patients' requirements, in relation to their goals. Plan, manage and review functional and psychological restoration for individuals and groups. Review care plans at timely intervals and communicate effectively with the patient's GP, advising on ongoing and flare up management plans agreed with the patient. Provide safe, effective and dignified care according to best practice and national guidelines. To provide advice and treatments to individual patients within best practice and national guidelines.
