About this role
Qualifications: High School Diploma/GED required: BA degree preferred. Must have two years experience working with people who have developmental disabilities and one year supervisory experience required. A valid NYS driver's license is mandatory and must be insurable under New Hopes's insurance regulations. Possession and maintenance of certification to administer medication is mandatory.
Essential Functions:
• 1. Meet with supervisor regularly.
• 2. Responsible for the day-to-day operations of the house and implementation of agency mission and goals.
• 3. Obtain/Maintain required certifications as outlined by Administration/Staff Development and Training.
• 4. Meet with the Service Coordinator, Nurse and Clinician on-going for both the coordination of the program planning and health needs of the individuals in the house.
• 5. Serve as an acceptable role model for the individuals within the house and will be held to a higher standard. Follow all agency policies and procedures to include teaching, monitoring and ensuring compliance.
• 6. Supervise and regularly communicate with and complete performance appraisals of all assigned staff.
• 7. Coordination and implementation of individual's objectives as outlined in their individualized plans.
• 8. Oversee the organization and maintenance of the house and ensure the follow-up of all maintenance requests.
• 9. Teach, train and educate all new staff in the functions of the house and provide ongoing training to existing staff including but not limited to abuse/mistreatment prevention training.
• 10. Responsible for the development of the staff schedule to ensure adequate coverage and adequate supervision on all shifts.
• 11. Ensure that all assessments appropriate for residential supports are completed in a timely manner.
• 12. Responsible for record keeping as follows:
• Ensure necessary logs are complete and accurate.
• Maintain up-to-date clinical and medical files in conjunction with Nurse and Service Coordinator.
• Ensure accurate financial records are maintained for each individual.
• Ensure incident reports are completed, reviewed, follow-up and filed within the individual's record.
• Ensure fire drills are conducted and maintain all fire safety documentation as required.
• Ensure that a mileage log and maintenance records are maintained for the house vehicles.
• Maintain data collection, goal sheets and attendance records.
• Supply inventories (i.e., house needs, clothing, hygiene supplies and personal possessions for all individuals).
• 13. Be on call 24 hours per day in the event of emergencies.
• 14. Responsible for house petty cash, food stamps, individual petty cash and all money within the house ensuring that receipts and accurate balances are maintained.
• 15. Ensure appropriate menu planning in conjunction with the Nurse and Dietician.
• 16. Supervise and ensure that all medication is ordered and delivered from the pharmacy. All medication is to be reviewed, including review of medication logs and checking the medication lock box.
• 17. Authority to hire and fire employees and makes recommendations regarding hiring, firing and advancement (such as reclassifications and promotions)
• 18. Maintain professional conduct when dealing with other agencies, families and community programs.
• 19. Perform hands-on direct care responsibilities as needed, which include various types of physical intervention, i.e., lifting or transferring of individuals.
• Conduct investigations as assigned by the Director of Corporate Compliance and meet as required with the Special Review Committee.
• Provide initial 30-day and quarterly or semi-annual summary updates as applicable.
• Maintain required trainings and documentation as required on the various
systems and logs provided
Additional Functions:
• 1. Coordinate all shopping trips.
• 2. Ensure that individuals are neatly dressed and groomed.
• 3. Provide staffing coverage as program indicates.
• 4. Ensure that recreational activities are scheduled and that goals are followed through.
• 5. Ensure all staff maintains required trainings within required time line.
• 6. Supervise light maintenance, including but not limited to: minor household repairs, snow removal, lawn care, vehicle care and maintenance and to keep the house neat and clean through the use of preventative maintenance checklist.
• 7. Keep all individual records and evaluations confidential.
• 8. Ensure that medical and clinical appointments are scheduled when necessary and appropriate follow-up occurs.
• 9. Plan and coordinate vacations for individuals.
• 10. Participate in the staff in-service training programs as required.
• 11. Carry out all other responsibilities deemed necessary to the position by the Assistant Director of Residential Services/Director of Residential Services.
Working Conditions/Physical Requirements:
• Occasional sitting, pushing, pulling, bending and squatting.
• Occasional need to lift up to 50 lbs independently and up to 100 lbs with assistance.
• Occasional need for physical intervention (SCIP).
• Potential for exposure to blood and body fluids.
• Occasional computer work.
• Frequent driving.
