RN Subacute Coordinator Salaried: $83,200 - $87,360 (depending on experience) Equivalent Hourly: $40- $42 Sign-On Bonus: $10,000!! Shift: Full-Time (40 hours/week) Schedule: Monday-Friday, 12 pm - 8 pm; rotating on-call schedule

Susquehanna Nursing & Rehabilitation Center is looking to add an Subacute Coordinator to its nursing leadership team. In this role, you'll oversee the daily operations of an assigned nursing unit, ensuring high-quality resident care, staff supervision, regulatory compliance, and exceptional customer service. RESPONSIBILITIES:

• Direct, coordinate, and supervise the activities of nursing staff on an assigned unit

• Assume responsibility and accountability for the overall management of resident care within the unit

• Monitor resident conditions and ensure care plans are developed, maintained, and updated as needed

• Collaborate with physicians, interdisciplinary team members, residents, and families to support quality care outcomes

• Communicate staff concerns and operational issues to appropriate department leaders

• Ensure compliance with facility policies, state and federal regulations, and nursing standards of practice

• Support staff development, coaching, performance management, and team engagement

• Assist with admissions, discharges, family communication, and resident care coordination

• Promote a safe, positive, and resident-centered environment

• Perform additional duties as assigned

REQUIREMENTS:

• Degree from an accredited School of Nursing

• Current and valid New York State Registered Nurse (RN) License required

• Minimum of one year of experience in nursing service administration required

• Additional experience in rehabilitative or geriatric nursing preferred

• Strong knowledge of state, federal, and local long-term care regulations

• Excellent leadership, communication, and organizational skills

• Strong customer service skills and a compassionate approach to resident care

• Must be flexible, dependable, professional, and able to manage multiple priorities
