Help Families Build Stability and Hope East Bay Community Action Program (EBCAP) is seeking a compassionate and resourceful Family Advocate - Housing to support individuals and families working toward stable housing and improved quality of life. Job Description The Family Advocate - Housing provides enrollment, outreach, advocacy, and case management services for EBCAP's housing stabilization programs. This position works directly with individuals and families to assess needs, develop goals, coordinate services, and connect participants with resources that support long-term housing stability and overall well-being. The role also supports grant-funded initiatives through data collection, reporting, community engagement, and program compliance activities. Case Management & Client Support Provide individualized case management services focused on housing stabilization and self-sufficiency. Conduct comprehensive assessments to identify client strengths, barriers, and service needs. Develop client-centered goals and action plans that support long-term stability. Complete home visits to assess living conditions and identify opportunities for additional support. Connect individuals and families with community resources, supportive services, and housing programs. Build trusting, respectful, and professional relationships with clients while maintaining appropriate boundaries. Community Outreach & Engagement Conduct outreach and recruitment activities to connect eligible individuals and families with services. Provide education and outreach regarding lead poisoning prevention and housing-related initiatives. Collaborate with internal programs and external partners to address client needs and increase access to services. Respond to referrals from community organizations and service networks. Data Management & Program Compliance Maintain accurate client records and case documentation. Enter and manage data within HMIS and other required systems. Assist with monthly, quarterly, and grant-related reporting activities. Ensure compliance with HUD, RIDOH, DHS, and agency requirements. Support data collection efforts that demonstrate program outcomes and effectiveness. Collaboration & Professional Development Participate in supervision, team meetings, and interdisciplinary collaboration. Attend trainings, conferences, and professional development opportunities related to housing stability and homelessness prevention. Work closely with EBCAP staff and community partners to coordinate services and improve client outcomes. Support special projects and additional program initiatives as assigned
