About this role

Job summary

An exciting opportunity has arisen to join the friendly and supportive team at Penn Hill Surgery as a Health & Wellbeing Coach on a 12-month fixed-term contractfor 18 hours per week to be worked on Tuesdays, Wednesdays and Fridays, 10am to 4pm. This role is jointly funded by Yeovil Primary Care Network (PCN) and Penn Hill Surgery and forms an integral part of our multidisciplinary team.

As a Health & Wellbeing Coach, you will empower patients to take greater control of their health and wellbeing through personalised coaching, behaviour change and self-management support. Working alongside GPs, nurses, pharmacists, social prescribing link workers and care coordinators, you will support patients with long-term conditions and lifestyle-related health needs to achieve goals that matter most to them.

We are looking for someone who is compassionate, motivated and passionate about delivering person-centred care. You will have excellent communication and organisational skills, enjoy building positive relationships and be committed to helping patients improve their health outcomes.

Main duties of the job

As a Health & Wellbeing Coach, you will work closely with patients to help them improve their health, wellbeing and confidence in managing their own care. Using a personalised, strengths-based coaching approach, you will support individuals to identify what matters most to them, set achievable goals and develop practical action plans that encourage long-term behaviour change.

Working as part of the multidisciplinary team, you will collaborate with GPs, nurses, pharmacists, care coordinators, social prescribing link workers and other healthcare professionals to ensure patients receive holistic, joined-up care. You will also signpost patients to appropriate community and voluntary sector services to support their health and wellbeing.

The role includes managing your own caseload, maintaining accurate patient records using EMIS Web, recording interventions and outcomes, and contributing to service evaluation and quality improvement. You will promote personalised care throughout the practice and work within Penn Hill Surgery and Yeovil Primary Care Network policies, ensuring high standards of confidentiality, safeguarding and patient care at all times.

This is an exciting opportunity to join Penn Hill Surgery as a Health & Wellbeing Coach on a 12-month fixed-term contract, jointly funded by Penn Hill Surgery and Yeovil Primary Care Network (PCN).

As a Health & Wellbeing Coach, you will play an important role in supporting patients to improve their physical, mental and emotional wellbeing through personalised coaching and behaviour change techniques. Working as part of our multidisciplinary team, you will empower patients to take greater control of their health by helping them identify what matters most to them, develop realistic health goals and build the confidence and skills needed to achieve lasting improvements in their health and wellbeing.

The successful candidate will work collaboratively with GPs, nurses, pharmacists and other healthcare professionals to provide holistic, person-centred care. The role focuses on improving patient outcomes, reducing health inequalities and promoting self-management, while ensuring patients are connected to the most appropriate services and community resources.

The role will include:

Delivering one-to-one and group health coaching sessions using a personalised, strengths-based coaching approach.

Supporting patients to identify what matters most to them, develop achievable goals and create personalised action plans.

Encouraging sustainable behaviour change and supporting patients to overcome barriers affecting their health and wellbeing.

Managing and prioritising your own caseload, ensuring patients receive timely support and appropriate follow-up.

Working collaboratively with the multidisciplinary team to ensure patients receive coordinated, joined-up care.

Signposting patients to appropriate community groups, voluntary organisations and local support services.

Maintaining accurate patient records using EMIS Web and recording interventions using the appropriate clinical coding.

Collecting outcome data and contributing to service evaluation, quality improvement and the ongoing development of personalised care services.

Promoting personalised care and supported self-management throughout the practice and wider Primary Care Network.

We are looking for someone who is compassionate, organised and motivated, with excellent communication and interpersonal skills. You will enjoy building trusting relationships with patients and colleagues and be committed to delivering high-quality, person-centred care. Experience of supporting behaviour change, coaching or working with people in a health, care or community setting would be advantageous, although applicants with transferable skills and a passion for helping others are encouraged to apply.

Person Specification

Experience Essential

Experience working in a health, social care, community or wellbeing setting, or another role involving direct support to individuals. Experience of supporting individuals to achieve personal goals or make positive lifestyle or behaviour changes. Experience of managing and prioritising a varied workload whilst working independently and as part of a team. Experience of building positive, professional relationships with patients, service users or members of the public. Experience of communicating effectively with people from a wide range of backgrounds, including those with differing health, social or emotional needs. Experience of maintaining accurate records and handling confidential information in accordance with data protection requirements.

Desirable

Experience working within General Practice, the NHS or a Primary Care Network. Experience delivering health coaching, wellbeing coaching or behaviour change interventions. Experience supporting people living with long-term conditions or multiple health needs. Experience of motivational interviewing or other recognised coaching techniques. Experience of working within a multidisciplinary team. Experience of signposting or referring individuals to community, voluntary or statutory services. Experience using EMIS Web or other electronic patient record systems. Experience delivering group sessions, workshops or health education programmes

Qualifications Essential

GCSE Grade 4/C or above (or equivalent) in English and Maths. A recognised qualification in Health & Social Care, Health & Wellbeing, Community Development, Coaching, Counselling or another relevant discipline or equivalent demonstrable experience in a health, social care or community setting. Willingness to undertake any training required to fulfil the Health & Wellbeing Coach role, including Personalised Care Institute (PCI) accredited Health Coaching training if not already completed.

Desirable

NVQ Level 3 (or above) in Health & Social Care or an equivalent qualification. Personalised Care Institute (PCI) accredited Health & Wellbeing Coach qualification. Qualification or accredited training in Health Coaching, Behaviour Change, Motivational Interviewing, Counselling or Coaching. Mental Health First Aid, Lifestyle Coaching or similar wellbeing-related qualification. Training in safeguarding, information governance and/or confidentiality. Current Basic Life Support (BLS) training.

General Requirements Essential

Able to work the required hours (18 per week) on Tuesday, Wednesday and Friday. Able to satisfy the requirements of an Enhanced Disclosure and Barring Service (DBS) check. Committed to undertaking mandatory training, including safeguarding, information governance and infection prevention and control. Committed to equality, diversity and inclusion and delivering person-centred care. Demonstrates professionalism, integrity and the ability to maintain confidentiality at all times. Willingness to undertake continuing professional development and complete Personalised Care Institute (PCI) accredited Health & Wellbeing Coach training if required. Able to work collaboratively as part of a multidisciplinary team while also working independently.

Desirable

Full UK driving licence and access to a vehicle if travel is required. Knowledge of local health, social care and voluntary sector services within Somerset. Previous experience of working within an ARRS-funded role or Primary Care Network. Flexibility to support occasional meetings or training sessions outside normal working hours, where reasonable.
