Job summary

Clarendon Medical Centre is looking for an enthusiastic and patient-focused Clinical Pharmacist to join our friendly and supportive multidisciplinary team. This is an exciting opportunity to play a key role in delivering high-quality care, improving patient outcomes and shaping the future of medicines optimisation within primary care.

Working closely with GPs, nurses and other healthcare professionals, you will provide expert clinical pharmacy services including structured medication reviews, long-term condition management, prescribing support and medicines optimisation. Main duties of the job

As a Clinical Pharmacist at Clarendon Medical Centre, you will play a central role in supporting patients to get the best outcomes from their medicines while working as part of a collaborative multidisciplinary team. You will provide expert clinical pharmacy services, undertake structured medication reviews, support patients with long-term conditions and complex medication needs, and contribute to improving the quality and safety of prescribing across the practice.

The role offers a varied and rewarding mix of direct patient care, clinical decision-making, medicines optimisation and service development. You will work closely with GPs, nurses, community pharmacies and other healthcare professionals to ensure patients receive safe, effective and evidence-based treatment.

You will also have opportunities to lead quality improvement initiatives, support clinical governance activities, contribute to enhanced services and play an active role in developing pharmacy services within the practice. This position is ideal for a pharmacist who enjoys building relationships with patients, working collaboratively across healthcare teams and making a meaningful difference to patient care within the local community.

Our clinical team consists of three GP Partners, two Salaried GPs, experienced Practice Nurses, Healthcare Assistants and a dedicated team of administrative and management staff.

The Clinical Pharmacist acts within professional boundaries as part of the general practice multidisciplinary team.

The role improves patient outcomes through medicines optimisation, structured medication reviews, prescribing support, long-term condition management and clinical leadership.

The post holder will contribute to patient safety, prescribing quality, service development and integration across primary care services.

Person Specification

Experience Essential

Minimum of two years experience working as a . Appreciation of General Practice and Primary Care. Understanding of primary care prescribing and medicines optimisation. Experience of acute and chronic conditions seen in GP.

Qualifications Essential

Enrolled in, or completed, an approved Clinical Pharmacist training pathway or equivalent. GPhC Registered Pharmacist. Independent Prescribing qualification, or actively working towards qualification
